Antimicrobial stewardship intervention: optimizing antibiotic treatment in hospitalized patients with reported antibiotic allergy

背景
入院患者において報告された抗菌薬アレルギーは、より多くの有害事象、代替抗菌薬の使用、および長期入院と関連するようである。抗菌薬アレルギーが報告された患者のほとんどで表示が打ち消される可能性があるため、抗菌薬管理介入を設定した。

目的
報告された入院患者の抗菌薬アレルギーが、抗菌薬治療に及ぼす影響を明らかにすること、および積極的な抗菌薬管理介入により、推奨される抗菌薬治療からの不必要な逸脱を避けること。

方法
2019 年 3 月から 5 月にオランダの教育病院において、抗菌薬アレルギーが報告された入院患者を介入研究の対象とした。医師は研修を受け、アレルギー表示を理由に推奨される抗菌薬治療からの不必要な逸脱があった場合、電子カルテ内に推奨事項を提示され、その患者を薬物負荷試験の対象とした。

結果
抗菌薬アレルギーが報告された患者 492 例を特定し、入院件数は 558 件を占めた。抗菌薬アレルギーの表示により、推奨される抗菌薬治療が妨げられたのは 93 例であった。これらの患者のうち 68 例が薬物負荷試験の対象となり、42 例が負荷試験を受けた。42 例中 40 例（95％）でアレルギー反応が認められず、推奨される抗菌薬治療が実施された。薬物負荷試験後に2 例（5％）で重度ではない皮膚反応が生じ、代替抗菌薬レジメンを継続した。

結論
抗菌薬アレルギーの表示が報告された患者に、推奨される抗菌薬治療を提供するために、そして抗菌薬再曝露で問題がなければ、表示を打ち消すために、この抗菌薬管理介入を使用することができる。

抗菌薬アレルギーの患者の割合を本論文から知ることができる。治療の効果を考えると、特定の抗菌薬を使用したくても患者の申告や診療歴から知りうる抗菌薬アレルギーの状況や負荷試験により程度を知ることにより、治療の選択肢が広がることになる。
