発展途上国に対する世界保健機関の主要な優先課題としての感染制御★

医療関連感染は世界中で何億人もの人々が罹患しており、患者の安全にとってのグローバルな主要問題である。先進工業国の急性期治療病院では、入院患者の5%~10%が医療関連感染症を合併している。発展途上国では、そのリスクは2~20倍も高く、感染率が25%を超えることも多い。この問題に対する認識の高まりを受けて、世界保健機関(WHO)は患者安全のための世界連盟(World Alliance for Patient Safety)を設立した。医療関連感染の予防は、2005年10月に当連盟内に発足したGlobal Patient Safety Challengeの第1回の主題である「Clean Care is Safer Care(清潔なケアはより安全なケア)」の目標とされている。その2年後、医療関連感染の抑制活動実施を支援する誓約として、公式声明に72カ国の保健衛生関連大臣が調印したが、このうち30カ国は発展途上国であった。さらに、発展途上国が大半ではあるが、2008年中の調印を予定している国々もあり、これらを併せると世界人口の4分の3以上に相当する。提案された戦略はシンプルで低コストの解決策に重点を置いていることから、発展途上国で高い効果が得られると考えられる。また、これによって期待される種々の効用の結果、利用可能な資源や発展の程度にかかわらず、あらゆる医療環境の基本的な感染制御策の改善を通じて何百万人もの生命を救い、何億人もの患者の罹患と長期障害を予防し、大幅なコスト削減をもたらす可能性がある。

監訳者コメント:
発展途上国において求められる感染対策は、物量に頼った大規模システムではなく、何はさておき、身の丈にあった一般衛生レベル(general hygiene level)の向上である。当たり前の衛生レベルが当たり前に維持されなければ、感染予防の前提が根底から崩れてしまう。このようなレビューを読んでおくと、限られた予算と人材がいない地域や病院では、最低限何を果たしていくべきかが明瞭にわかるので、ぜひ一読をお勧めする。
