Norovirus in the hospital setting: virus introduction and spread within the hospital environment

2009 年から 2010 年にかけての 4 か月間に、胃腸炎症状を呈する入院患者からノロウイルス株が検出された。市中から病院内に持ち込まれたノロウイルスの株数を推定するために、採取したノロウイルスの特性を評価した。さらに、感染患者が滞在している病室・病棟の清掃後に、環境のスワブ採取を実施した。このスワブ採取の目的は、清掃の効果を評価すること、およびノロウイルスによる環境汚染を判定することである。この 4 か月間に、GII-4 型ノロウイルスの合計 8 種類の遺伝子群が同定され、このシーズン中に一部の病棟で異なる GII-4 株による複数のアウトブレイクが発生していた。清掃後の環境スワブの 31.4％にノロウイルスが検出された。ノロウイルスのリザーバとして、書類用カート、コンピューターのキーボード、石けん・アルコールのディスペンサー、血圧測定装置、パルスオキシメーター、および鼓膜体温計が特定されたが、病床周囲の環境表面、家具、作り付け設備、トイレ・浴室の付属器具からも汚染が検出された。ウイルス特性の詳細な評価と環境のスワブ採取の併用は、アウトブレイクの規模や範囲を判定し、臨床現場での清掃の効果をモニターするための感染制御監査の強力な手段となる。

ノロウイルス感染症の院内伝播とウイルス株の関連を調べた研究。多数の環境検体中からノロウイルス遺伝子が検出されたことは、より念入りな環境清掃が必要であることを示唆している。一方で、ウイルス株の同定自体は学問的には興味深いが、その結果が臨床現場での感染対策にどのように役立つのかが見えてこない。
