Nosocomial meningitis in a university hospital between 1993 and 2002

本研究の目的は、院内感染による髄膜炎（以下院内感染髄膜炎）と、外科的処置、病原体の種類、および他の病院感染症との関係を明らかにすることである。Ankara大学Ibn-i Sina病院の脳神経外科において、1993年から2002年の間に米国CDCの基準に従って院内感染髄膜炎と診断された51例の患者を後向きに評価した。院内感染髄膜炎の全患者は集中治療室に入院した。外科的予防のためには第三世代セファロスポリン薬が使用されており、治療のために広域抗生物質が使用されていた。院内感染髄膜炎は全入院患者の0.34％に発症し、全病院感染症の0.53％を占めた。14例（28％）が少なくとも１つの病院感染症を併発しており、主に手術創に由来した感染症や二次性菌血症を認めた。4例が手術部位感染症の後に同一の原因菌による院内感染髄膜炎を発症し、3例が菌血症を発症した。患者はすべて外科的処置を受けており、26例（51％）は脳室腹膜短絡術を受けていた。49例の患者の脳脊髄液で原因微生物が認められ、うち16例は多菌種の原因微生物を有していた。全67の微生物分離株のうち、グラム陰性桿菌は41（61％）であり、グラム陽性球菌は23（34％）、残りはカンジダ属が3（5％）であった。最も多くみられた病原菌はブドウ球菌（30％）であり、次は非発酵性グラム陰性桿菌（22％）であった。
