病院における空気消毒法としての乾熱の概念検証評価:殺菌効果に関する予備的データ★

背景

病院では、医療関連感染症の予防のために患者の身近な環境のバイオクリーニングが必要である。微生物は、特定の環境表面において最高数か月間という長期間にわたり生存することができる。その結果、直接の接触や医療従事者の手指を介して患者にそれらの微生物が伝播することになる。乾熱は、古くから抗菌処理法として認められている。しかし、病院表面に対する空気消毒法としての使用は新しい。この消毒法が現行の標準を満たしているかを明らかにするために、実臨床に適用する前に殺菌効果を評価する必要がある。

目的

本研究は、60°C の温度で生成した乾熱による 30 分間の消毒が室内の水平表面の細菌汚染に及ぼす効果を評価し、同法を現行の標準法である洗浄消毒剤の直接塗布による消毒と比較することを目的とした。

方法

病室でごく普通にみられる材料で作成した 4 つの支持台に、病院の生態系を代表する 5 種類の細菌を播種して試験を行った。消毒フェーズ後の異なる時点に 900 個以上の細菌学的サンプルを採取した。

結果

乾熱処理の殺菌活性は、細菌種または材料の違いにかかわらず、10 分間で 5 log10 cfu/mL を超える細菌レベルの減少により示された。30 分後、洗浄消毒剤および機械のみによる場合よりも有意に高い効果が得られた。

結論

乾熱(60°C で 30 分間)は、本研究で試験を行った条件下において、非芽胞形成性栄養細菌に対して殺菌活性を示した。本法は自動化が可能で、生態学的基準を満たし、ヒトに対して毒性をもたない。本法は、栄養細菌に対する四級アンモニウム消毒剤を用いた表面消毒の代替法となる可能性があるが、臨床的に重要な他の病原体に対する有効性を確認するためにさらなる研究が必要である。

監訳者コメント :

環境整備において、環境表面の消毒は現在、第四級アンモニウム塩が主流である。しかしながら、qac 遺伝子による消毒薬耐性や抗菌薬の交差耐性と同時に、排水系や環境での生分解性が低いことでバイオフィルムへの蓄積による菌への選択圧も懸念される。一方、乾熱による殺菌はこれらのデメリットがなく、本論文はまだ実環境での検証がないものの、さらなる実証データを集めるとともに、使用条件や使用範囲などを工夫することで有望な環境整備の手段になるかもしれない。
