Floor and environmental contamination during glove disposal

背景
これまでのエビデンスによって、使用した個人防護具（PPE）の取り外し時と、おそらくは廃棄時に環境が汚染される可能性が示唆されている。
目的
医療用手袋が不適切に廃棄される際の、周辺および床の汚染の可能性を確認すること。
方法
バクテリオファージおよび化学染料を接種した手袋を、医療従事者 15 名が 1.22 m 離れた場所にあるごみ箱に捨てた。各試験後に、あらかじめ定めておいたサンプル区域について、ブラックライトを使って蛍光染料による染まりの有無を目視で確認するとともに、バクテリオファージを定量する目的で 3M 社のレーゼンブロススポンジで拭き取った。
結果
医療従事者に最も近い区域（0.30 m未満）では、バクテリオファージの濃度が最も高かった（相乗平均 6.9 × 10³ pfu/100 cm²、範囲 8.07 ～ 3.93 × 10⁷ pfu/100 cm²）。医療従事者から 0.61 m 超の区域と比較して、0.61 m 以下の区域ではバクテリオファージの濃度が有意に高かった（P ＜ 0.05）。最も遠い距離（1.22 ～ 1.52 m）ではバクテリオファージ陽性率は 14％、蛍光染色陽性率は 4％であったが、これら2つのトレーサー間の有意差はなかった（P ＝ 0.069）。バクテリオファージ、化学染料の両方で、環境汚染は医療従事者の近くで最も大きいこと、ならびにこれらのトレーサーは PPE 廃棄に関するトレーニングに適していることが示された。
結論
医療従事者は日常的に手袋を使用しており、不適切に廃棄すればその際に周囲の環境表面や床を汚染する可能性がある。したがって病原体の伝播を最小限にするため、業界全体の方針の確立、十分なトレーニングおよび医療従事者への教育による、適切な廃棄法が必要とされる。

監訳者コメント：
手袋を脱ぎ、廃棄する際に、汗とともに菌が飛散することは容易に想像できる。それを示した本検討はユニークであり、感染制御の本質的な部分を捉えてもいる。本研究のデータをもとにした今後の現実的な体制の構築と、本テーマに関するこれからの研究の進展が待たれる。
