背景
緑膿菌(Pseudomonas aeruginosa)は集中治療室(ICU)における主要な院内感染病原体であるが、汚染の由来が内因性か外因性かについては依然として明らかではない。
目的
ICU における緑膿菌獲得の患者および環境のリスク因子を特定すること。
方法
フランスの ICU 10 施設で 5 か月間の前向き多施設研究を実施した。ICU に 24 時間以上入室した成人患者を対象として、入室時、入室中週 1 回、および退室前に緑膿菌保菌のスクリーニングを行った。緑膿菌獲得の定義は、入室時にスクリーニングスワブ陰性で、その後に保菌・感染が認められた場合とした。ICU の水道から水サンプルを週 1 回採取した。患者特性、侵襲性器材の使用、抗菌薬療法、水および患者の緑膿菌保菌圧、および ICU 特性に関するデータを収集した。多変量 Cox モデルを用いてハザード比(HR)を推定した。
結果
ICU 入室時に緑膿菌保菌・感染がない 1,314 例のうち、201 例(15%)が入室中に緑膿菌を獲得した。緑膿菌獲得に有意に関連する患者特性は、以前の緑膿菌感染・保菌歴、人工呼吸器累積装着期間、および緑膿菌に対する活性がない抗菌薬の累積投与日数であった。緑膿菌獲得の環境リスク因子は、病棟の 1 日あたりの「nine equivalents of nursing manpower use score(NEMS)」の累積(30 ポイント以上のハザード比[HR]1.47、95%信頼区間[CI]1.06 ~ 2.03)および病室の水道水汚染(HR 1.76、95%CI 1.09 ~ 2.84)であった。
結論
緑膿菌獲得の介入可能な患者および環境のリスク因子が特定された。

監訳者コメント
本論文では、緑膿菌獲得のリスクとして、患者個人に起因するリスク因子が環境に起因するリスク因子よりもハザード比が高かった。そのうえで、環境因子として、水の汚染とスタッフの業務負担がリスクであるとしている。水道管への対応とともに、アルコールによる手指衛生の推奨も必要であろう。リスクアセスメントのためのリスク分類の重要性を考えることができる論文である
