排泄物管理と基質特異性拡張型β‐ラクタマーゼ産生腸内細菌科細菌の発生率との関連:医療従事者の知識と実践の影響★

背景
近年、医療環境における基質特異性拡張型β‐ラクタマーゼ(ESBL)産生腸内細菌科細菌の伝播は、公衆衛生上の深刻な脅威となっている。
目的
医療従事者が排泄物をどのように管理しているか、ならびに、その管理と ESBL 産生腸内細菌科細菌の発生率との関連の可能性を評価すること。
方法
74 の医療部門における医療従事者 800 名および管理看護師 74 名を対象に、知識および実践について評価するため、ならびに、排泄管理に利用する備品を確認するために、2 種類の自記式質問票を用いて調査した。利用される備品、知識、および実践に関する成績について、事前に設定された閾値に基づき、良(スコア 1)、中等度(スコア 2)、不良(スコア 3)とスコア化した。線形回帰を用いて、医療従事者の知識・実践と ESBL 産生腸内細菌科細菌の発生率との関連を評価した。
結果
医療従事者 688 名(86%)と管理看護師全員が調査に参加した。スコア 1、2、3 とした回答者の割合は、備品については、それぞれ 14.8%、71.6%、17.6%、知識について 30.1%、40.6%、29.3%、実践について 2.0%、71.9%、26.1%であった。単回帰数学モデルでは、医療従事者の実践の不良(スコア 3)が、ESBL 産生腸内細菌科細菌の発生率の増加と有意に関連した(P = 0.002)。
結論
医療従事者による排泄物管理の実践とESBL産生腸内細菌科細菌の発生率との間に正の相関がみられ、とくに外科病棟において顕著であった。多剤耐性菌の伝播をより制御するためには、医療従事者の知識と実践を強化する公衆衛生上の取り組みの策定が急務であり、この取り組みを感染制御プログラム内に統合すべきである。

監訳者コメント:
ESBL 産生菌の発生率と排泄物処理に関する医療従事者の知識と実践について調査した論文である。まとめると、適切な処理方法を理解していないことは ESBL 産生菌の発生率を上げる、ということになるが、論文の結果に掲載されている個々の因子は、日本においても、検討すべき課題であると思われた。
