Asymptomatic carriage of toxigenic Clostridium difficile by hospitalized patients

無症状のクロストリジウム・ディフィシル（Clostridium difficile）保菌は病院内で多く認められるが、保菌者からの伝播リスクについては不明である。無症状の入院患者を対象とした今回の点有病率培養調査の結果、149 例中 18 例（12％）が毒素産生性 C. difficile 保菌者であった。無症状の保菌者では、C. difficile 感染患者と比較して皮膚および/または環境汚染率が有意に低かった（18 例中 3 例［17％］対 6 例中 5 例［83％］、P ＝ 0.007）が、保菌者数は C. difficile 感染患者よりも 3 倍多かった。これらのデータから、無症状の保菌者は病院内の C. difficile 伝播に寄与している可能性が示唆される。

監訳者コメント：
感染患者の 3 倍の保菌患者が存在し、少なからず環境汚染に関与している、という論文である。疑ったら C. difficile 感染患者として感染対策を実行する。ノロウイルス感染症も同様であろう。肝に銘じておきたい。
