Hospital-based epidemiology: a strategy for ‘dealing with Clostridium difficile

クロストリジウム・ディフィシル(Clostridium difficile)関連下痢症は、依然として感染制御における重大な問題である。その診断法および患者への感染源を特定する上での診断基準は確立していない。英国の大規模教育病院の検査室でC. difficile関連下痢症と診断された275例の新規症例(一般開業医の患者と病院入院患者)を対象として、その局地的な疫学の詳細を明らかにするため前向き疫学研究を実施した。最も発生率が高かった部門は血液内科と重症管理部門であり、またC. difficile関連下痢症の院内発生例の29%は、65歳未満の患者に発生しており、その割合は意外に高かった。入院後48時間以内にC. difficile関連下痢症と診断された症例は55例であった。この55例のうち「市中獲得型」とされた症例数は、感染した場所と下痢の発生時間に関する種々の基準によって9例から25例までばらつきがあった。同様に、市中発生症例48例のうち「市中獲得型」とされたのは19例から25例であった。このようなばらつきがみられたことから、国民保健サービス(National Health Service;NHS)トラスト間の比較は不正確なものであると考えられる。症例を診断から90日間追跡し、全原因死亡のデータを収集した。病院入院後に診断された227例中56例(25%)が30日以内に死亡し、このうち29%が65歳未満であった。死亡例の86%から死亡証明書のデータが入手できた。15例(31%)の証明書にC. difficileの記載があり、8例(17%)ではC. difficileが主要な死因(1aまたは1b)とされていた。今回の研究は、NHSトラスト内で感染の予防・制御戦略を策定するため、および国レベルでの標的と政策のエビデンスの基盤をもたらすために、局地的な疫学研究が有用であることを示している。

C. difficile感染症の疫学研究の遅れは、症例定義が定まっていないために、有病率や死亡原因の特定が困難であるという背景がある。早期の症例定義の確立が望まれる
