Emergence of carbapenem-resistant Enterobacteriaceae in a UK paediatric hospital

背景
カルバペネム耐性腸内細菌科細菌は、世界的な感染症の脅威をもたらしている。しかし、小児での臨床的重要性を示したデータはほとんどない。
目的
この後向き研究では、英国の大規模 3 次紹介小児病院の臨床サンプルおよびサーベイランスサンプルから培養したカルバペネム耐性腸内細菌科細菌の保菌率および耐性機序について調査した。
方法
2011 年 9 月から 2012 年 8 月にリバプールの Alder Hey Children’s NHS Foundation Trust に提出された診断およびサーベイランス用サンプルを対象として、カルバペネム耐性腸内細菌科細菌の検索を行った。表現型解析または分子的方法により耐性機序を特定した。Variable number tandem repeat 法によるプロファイルを用いてカルバペネマーゼ産生株のタイピングを行った。
結果
12 か月の研究期間中に、患者 24 例からカルバペネム耐性腸内細菌科細菌を分離した。このうち 5 株は診断用の臨床サンプル由来であり、患者 421 例中の 19 例（4.5％）がサーベイランスのための直腸スワブ検査で陽性であったものである。分離株 24 株中 7 株（いずれもクレブシエラ［Klebsiella］属菌）はカルバペネマーゼ産生菌（bla_KPC 3 株、bla_NDM 4 株)、17 株は AmpC または基質特異性拡張型 β-ラクタマーゼ活性と薬剤不透過性を併せ持つ耐性であった。
結論
カルバペネム耐性腸内細菌科細菌、特にカルバペネマーゼ産生菌は、英国の大規模小児病院で感染症の問題を引き起こしている。これらの菌の拡散をモニターおよび制御するためには積極的サーベイランスが必要である。

Variable number tandem repeat 法は、従来の PFGE 法に比べ簡便かつ迅速に結果が得られるのが特徴の遺伝子型別法である。結果の数値化が可能であるため，検査施設間での比較が可能となると期待されている。一般の病院では、遺伝子解析は、その複雑な手法だけでなく設備や費用の障害もあり、アウトブレイク以外では、まだまだ身近な検査法ではなく、積極的なモニターに使用できる方法が出てきてほしいところである。
