2013 年以来、開胸心手術時のマイコバクテリウム・キマイラ(Mycobacterium chimaera)伝播においてヒータークーラーユニットが果たす役割が認識されるようになった。その後の研究により、関与するヒータークーラーユニットの広範な普及を反映した顕著な世界的アウトブレイクが明らかにされた。ヒータークーラーユニットは心肺バイパス手術の不可欠な要素であり、その撤去は生命を救う心臓手術の限界に重大な影響を及ぼすことになる。しかし、複数の研究により、多くのヒータークーラーユニットが広範な微生物(M. chimaera など)やバイオフィルムにより汚染されていることが示されている。あるメーカーのヒータークーラーユニットから全世界で回収された M. chimaera 分離株の全ゲノムシークエンシングにより、高いレベルの遺伝的類似性が示され、これに対するもっとも妥当に見える仮説は、共通感染源としてのデバイスのコンタミネーションである。ヒータークーラーユニット内の水タンクの破損部からのバイオエアロゾルの拡散は、最も考え得る伝播経路であり、手術室で超清浄換気を使用していたとしても空気感染細菌が手術野に達していることが示されている。ヒータークーラーユニット内を循環する水の微生物学的な質を調節すること、またバイオフィルム形成を抑制することは、これまで多くの病院にとって大きな課題であった。しかし、コンタミネーション除去戦略の強化は、これまで複数のメーカーにより推奨されており、ヒータークーラーユニットにおける M. chimaera の根絶において必ずしも有効ではないものの、英国の病院からこれらの制御戦略の実施後にM. chimaera 感染症の新規症例の報告はない。病院内の水安全チームは、ヒータークーラーユニットなどの医療機器には、致死的となり得る水系感染症の伝播の媒介物となる可能性があることに注意すべきである。

体外循環を用いての大血管系手術の際に、温度をコントロールする冷温水装置の 1 社の製品に非結核性抗酸菌が汚染している状態で供給されたたため、全世界的にこの機器が使用されている手術現場が非結核性抗酸菌症発症のリスクにさいなまれている。国内にも同社の製品はかなりの台数が使用されており、善処が求められるところである。
