肺炎桿菌(Klebsiella pneumoniae)による病院感染菌血症患者の転帰に対するカルバペネム耐性の影響

背景
カルバペネム耐性腸内細菌科細菌、特にクレブシエラ(Klebsiella)属菌は近年、世界的に健康上の重大な問題となっている。イスラエルにおけるカルバペネム耐性肺炎桿菌(carbapenem-resistant Klebsiella pneumoniae;CRKP)感染は、2006 年以降、大幅に増加している。これらの菌株を原因とする血流感染症では、治療不成功率や死亡率が高い。

 

目的
本研究は、医療関連 K. pneumoniae 菌血症患者におけるカルバペネム耐性のリスク因子、および医療関連 CRKP 菌血症による死亡の予測因子を、カルバペネム感性 K. pneumoniae(carbapenem-susceptible K. pneumoniae;CSKP)との比較によって特定するためにデザインされた。

 

方法
この後向き症例対照研究では、2006 年から 2008 年の K. pneumoniae 菌血症の全症例を特定し、CRKP 菌血症患者と CSKP 菌血症患者の耐性パターン、基礎疾患、薬剤耐性のリスク因子、および死亡率を比較した。

 

結果
CSKP 菌血症患者 214 例と CRKP 菌血症患者 103 例を比較した。重度の併存疾患、慢性併存疾患、および抗菌薬使用歴は CRKP 菌血症患者に多く認められた。多変量解析では、マクロライド系薬の使用歴および 14 日以上の抗菌薬投与が、CRKP 菌血症に関連する唯一の独立因子であった。CRKP 菌血症患者の死亡率は CSKP 菌血症患者よりも有意に高かった。多変量解析では、寝たきり状態、慢性肝疾患、Charlson 併存疾患指数 5 以上、人工呼吸器の使用、および血液透析が、K. pneumoniae 菌血症患者の死亡と独立して関連していた。カルバペネム耐性は死亡のリスク因子ではなかった。

 

結論
抗菌薬使用歴は CRKP 菌血症のリスク因子である。K. pneumoniae 菌血症患者の死亡は重度の併存疾患と関連するが、カルバペネム耐性とは関連していなかった。
