Impact of changes in Clostridium difficile diagnostic testing on detection of C. difficile infection and all England mandatory surveillance data

クロストリジウム・ディフィシル（Clostridium difficile）検査キットの性能不良を示唆する Centre for Evidence-based Purchasing の報告を受け、英国保健省は 2 段階の検査アルゴリズムを推奨する改訂指針を発表した。本研究の目的は、英国国民保健サービス（NHS）の微生物診断検査室を対象として電子メールを用いた質問票調査を実施し、指針改訂に基づいた検査法の変更について調査すること、またこれらの変更が国や地域からの報告データに及ぼす影響をモデル化することである。検査法を変更した検査室は 24％であることが判明し、検査性能の管理に用いられる英国の義務的サーベイランスのデータ全般への影響を示すエビデンスは得られなかった。また、検査法の変更が個々の NHS トラストの症例数に影響を及ぼした可能性が示され、トラストにおけるこのような C. difficile の臨床検査診断結果の変化を予測するための簡易モデルを開発した。さらに、サンプルの選択基準にばらつきがあることを示すエビデンスが得られ、このことが地域の検査の陽性的中率および陰性的中率に影響していると考えられた。

監訳者コメント：

検査方法が変更になれば当然、疫学も大きく変化する可能性がある。我が国では検査そのものを行えていない医療機関もまだまだ多い。
