Barriers to investigating and reporting nosocomial outbreaks to health authorities in São Paulo, Brazil: a mixed methods approach

背景
院内アウトブレイクがすべて保健当局に報告されているわけではない。
目的
院内アウトブレイクを調査し保健当局に報告することを妨げる障壁を特定すること。
方法
収斂的並行デザインを用いて混合研究法を実施した。この研究の量的および質的な部分は、それぞれ、州全体での（電子質問票）調査およびフォーカスグループである。ブラジルのサンパウロ州で働く感染管理担当者を登録した。
結果
感染管理担当者 85 名を調査に組み入れ、22 名をフォーカスグループに組み入れた。院内アウトブレイク調査および報告を妨げる障壁として以下があげられた。（1）アウトブレイク調査の知識を実践に移す際の困難、（2）アウトブレイク調査の過程での計画不十分、（3）組織文化および背景、（4）報告に関する認識不足、（5）保健当局にアウトブレイクを報告するという感染管理担当者の自主性不足。
結論
保健当局は、保健サービス機関と協力する自らの戦略を見直し、また院内アウトブレイク調査に対する知識と技能の改善を支援するトランスレーショナル教育プログラムを提供することで、これらの障壁を克服できるであろう。

行政の役割は処罰することではなく、市民が安心・安全な医療が受けられるように監督・指導をすることが先決である。日常的な情報交換やアウトブレイク時には迅速な連携により見守り体制を構築することが重要であろう。
