Should central venous catheters be rapidly removed to treat Staphylococcus aureus related-catheter bloodstream infection (CR-BSI) in neonates and children? An 8-year period (2010-2017) retrospective analysis in a French University Hospital

カテーテル関連血流感染症の治療は、カテーテルの抜去の有無に関わらず、経験的な抗菌薬治療に基づいている。本研究の目的は、黄色ブドウ球菌（Staphylococcus aureus）によるカテーテル関連血流感染症を有する新生児および小児において、カテーテルを速やかに抜去する、または速やかに抜去しない場合の不成功の発生について比較することである。治療不成功は、血液培養陽性の持続、局所／全身性の合併症の発症もしくは悪化、または再発と定義した。患者 225 例におけるカテーテル関連血流感染症 54 例（保存的治療 33 例、非保存的治療 21 例）について分析し、不成功は、それぞれ、10 例、3 例であった（P＜ 0.002）。カテーテルを速やかに抜去する非保存的治療は、不成功率の有意な低下と関連するようであり、推奨すべきである。

監訳者コメント：
黄色ブドウ球菌菌血症において、中心静脈カテーテルを温存した群は、抜去した群に比べて持続菌血症や再発などが多かったとする論文である。しかし症例数がそれぞれ 33 例および 21 例と少なく、またそれぞれの臨床背景も評価されていないため、この結果をもって新生児でも黄色ブドウ球菌菌血症で速やかに中心静脈カテーテルを抜去すべきであると結論付けるのは早急だろう。
