目的

高体重の患者において、体重に基づく周術期予防的抗菌薬の投与は議論の余地がある。体重 80 kg 以上の患者を対象に、2回(2 g)投与と単回投与(1 g)により消化管手術後の手術部位感染症(SSI)が減少するかどうかを比較評価した。

患者

体重 80 kg 以上で、2019 年 1 月 1 日から2025 年 5 月 31 日までに当施設で全身麻酔下の消化管手術を受け、周術期予防的抗菌薬としてセファゾリン、セフメタゾール、またはフロモキセフのいずれかの投与を受けた患者

方法

患者を単回投与群と 2 回投与群に分けた。SSI は米国疾病管理予防センターの基準に従って定義し、関連する共変量について 2:1 の傾向スコアマッチング後に比較した。

結果

マッチング後(単回投与 N = 100、2 回投与 N = 50)のベースライン特性は均衡していた。単回投与群の 15%、2 回投与群の 2%に SSI が発生した。2 回投与は SSI の減少と関連していた(オッズ比 0.117、95%信頼区間 0.003 ~ 0.804、P = 0.022)。SSI 減少の可能性は、ロジスティック回帰分析と下部消化管手術分析の両方で一貫していた。

結論

体重 80 kg 以上の患者には 2 回投与が有益である可能性がある。本研究は単施設の後向きデザインであったため、有効性を確認するためにはより大規模な研究が必要である。
