帰国後の医療ツーリストにおける手術部位感染症:知識ギャップの解消と改善点の特定★

背景
医療ツーリズム、特に美容外科手術や肥満手術のための医療ツーリズムはますます増加している。このことは、管理を要する術後感染症に罹患した状態で患者が帰国した場合、大きな問題となる。手術部位感染症(SSI)の治療ガイドラインは、通常、各国の抗菌薬感受性パターンに基づいており、抗菌薬耐性プロファイルや診療手順が異なる外国で手術が実施された場合には適切ではない可能性がある。
目的
本研究の目的は、国外で美容外科手術または肥満手術を受けた後にアイルランドの病院を受診した患者の人口統計学的特性、リスク因子、外科的管理、微生物学的原因、および SSI 治療について検討することであった。
方法
アイルランド内 4 つの 3 次形成外科紹介センターで前向き多施設観察研究を実施した。各参加病院で倫理承認を得、標準化されたデータ収集フォームにより報告の一貫性を確保した。米国疾病管理予防センターの基準に従って SSI を分類した。国外で美容外科手術または肥満手術を受けた後に感染症に罹患し、アイルランド内で受診した成人を適格とした。さらなる洞察を得、本研究の知見を補完するため、本患者コホートの様々な側面について形成外科医への調査も行った。
結果
本研究の対象となった患者 37 例中 34 例が美容外科手術、3 例が肥満手術を受けた患者であった。21 例(57%)がトルコで手術を受け、最も多い美容外科手術は腹部形成術であった(17 例、46%)。大半(34 例、92%)は「清潔」手術に分類される手技であった。22 例(59%)で明らかな微生物培養陽性が確認され、そのうち15例(68%)でグラム陰性菌が分離された。17 例(77%)では、分離株が各国の経験的治療ガイドラインで推奨される抗菌薬に耐性を示した。入院期間は 1 日から 107 日までであった。
結論
医療ツーリズム後の SSI は、各国の医療システムに問題をもたらす可能性があり、広範な原因病原体が関係しているため、本患者コホートには各国の既存の経験的治療ガイドラインが不適切であることが多い。広報キャンペーン、最新の傾向および合併症の正確な記録を可能にするためのサーベイランスの強化、そして本患者コホートに対する経験的治療ガイドラインの更新のすべてが、患者の転帰を最適化するために不可欠である。
