背景

造血幹細胞移植(HSCT)は多くの悪性および非悪性血液疾患に対する重要な治療法であるが、レシピエントは重度の感染症、特に敗血症に罹患しやすくなる。

方法

本後ろ向き横断研究では、2016 年から 2022 年までの米国の全国入院患者サンプルを用いて、HSCT による入院における敗血症の有病率および関連因子を評価した。国際疾病分類第 10 版臨床修正版の手技および診断コードを用いて、HSCT による入院を 25,409 件特定した。多変量ロジスティック回帰分析により関連因子を決定した。

結果

研究サンプル中の HSCT による入院における敗血症の有病率は 10.97%であった。敗血症の関連因子には、神経障害(オッズ比[OR]3.57、95%信頼区間[CI]1.98 ~ 6.42)、電解質障害(OR 2.40、95%CI 1.33 ~ 4.33)、うっ血性心不全(OR 2.30、95%CI 1.28 ~ 4.14)、栄養不良(OR 1.86、95%CI 1.03 ~ 3.34)、および麻痺(OR 1.83、95%CI 1.02 ~ 3.30)が含まれた。病院地域も敗血症のオッズ低下と関連していた。

結論

HSCT による入院における敗血症の有病率は 10.97%であった。神経障害、電解質障害、うっ血性心不全、栄養不良、および麻痺が敗血症と有意に関連していた。これらの知見は高リスク患者集団を特定するのに役立ち、HSCT における敗血症リスクおよび予防戦略を検討する今後の研究に有益となる可能性がある。
