整形外科の大手術中の細菌による手術野の汚染:これは心配の種か?★

背景
整形外科手術中の手術用吸引器の先端が細菌によって汚染されることは、微生物に術中曝露されていることを示している可能性があるが、それが手術部位感染症(SSI)の予測に役立つかどうかははっきりしていない。
目的
この研究の目的は、整形外科の大手術における吸引器先端の汚染の発生率、その SSI との関連、および関係するリスク因子について評価することであった。
方法
この前向きコホート研究には、大規模な 2 次医療病院で整形外科の大手術を受ける 760 例の患者を組み入れた(2022 年 2 月から 2024 年 3 月)。吸引器先端の断片を、術後無菌的に収集し、培養した。SSI を 90 日以上監視した。二項ロジスティック回帰で、汚染、SSI、リスク因子の間の関連を検出した。
結果
吸引器先端の汚染は、手術例の 3.55%に発生し、SSI は、手術例の 0.66%に発生した。汚染と SSI の間に有意な関連は見出されなかった。脊髄手術は、汚染(オッズ比[OR]4.485、95%信頼区間[CI]1.989 ~ 10.113)と SSI(OR 10.810、95%CI 1.783 ~ 65.555)両方のオッズを増加させた。手術時間が長いこと(OR、1.030、95%CI 1.011 ~ 1.049)、American Society of Anesthesiologists(ASA)スコアが 3 であること(OR 0.136、95%CI 0.023 ~ 0.823)、体格指数が高いこと(OR 1.197、95%CI 1.053 ~ 1.360)、免疫が抑制されていること(OR 10.236、95%CI 1.089 ~ 96.213)が、SSI のリスクの増加と関連していた。
結論
吸引器先端の汚染は、珍しくないが、SSI を予測するものではない。脊髄手術、長い手術、患者の併存疾患の方が、SSI の予測因子としては強い。吸引器先端の培養は、環境の汚染を示している可能性があるが、SSI の独立した予測因子としては、臨床であまり役に立たない。予防は、特に高リスクの手術においては、多因子性のリスクを標的とすべきである。
監訳者コメント :
吸引管先端培養は術中環境汚染の指標にはなりうるが、患者 SSI の予測因子としての臨床的価値は限定的である。
