緒論

カンジドザイマ・アウリス(Candidozyma auris、旧称カンジダ・アウリス[Candida auris])は、医療関連感染症の新たな原因菌であり、世界的に検出率が増加しており、重大な臨床的意味を持っている。2023 年 9 月以降、C. auris アウトブレイクが London National Health Service Trust で発生したため、その保菌または感染のリスク因子を明らかにするために後向き症例対照研究を実施した。

方法

2023 年 10 月から 2024 年 7 月に C. auris の検査を受けた患者のマッチド症例対照研究を実施した。症例(N = 76)を対照者(N = 152)と、診療科、年齢群、および性別により 1:2 の比でマッチさせた。人口統計学、併存症、手技、および抗菌薬使用に関するデータを、電子カルテから抽出した。ロジスティック回帰を用いて独立リスク因子を明らかにした。

結果

C. auris 保菌は、検査前の入院期間延長(1 日当たりオッズ比[OR]1.02)、高血圧(OR 2.9)、骨髄炎(OR 10.0)、ドプラプローブ使用(OR 3.8)、および抗菌薬曝露(OR 8.4)と関連した。留置デバイスとの有意な関連は認められなかった。保菌は、死亡率増加または入院期間延長と関連しなかった。

考察

本研究の結果は、環境衛生と早期スクリーニングの重要性を浮き彫りにしている。ドプラプローブの使用は、新たな伝播のベクター候補であることが認められた。これらの結果は、対象を絞った感染制御策を支持し、C. auris アウトブレイクの管理において先を見越したサーベイランスの価値を高く評価している。

監訳者コメント

ドプラプローブの使用が C. auris 陽性と関連していたが、病原体の供給源として確定されたわけではなく、皮膚に接触する共用機器が、清拭の不徹底、ポケット内保管、手指衛生や手袋交換の不足を介して媒介物となった可能性を示している。ICT ラウンドでは、消毒薬の種類だけでなく、「誰が、いつ、どの面を、どの手順で清拭し、どこに保管するか」まで確認する必要がある。特に長期入院患者では、入院時陰性でも新規獲得し得るため、アウトブレイク時には反復スクリーニングと共用機器の専用化・清拭監査を組み合わせることが重要である。
