内視鏡の用手洗浄を標準化する:AquaTYPHOON 装置の効果を評価する

背景
内視鏡的処置は、多くの場合内部チャネルの不十分な洗浄に起因する感染のリスクを伴う。用手洗浄は不可欠であるが、ヒューマンエラーが起きやすい。このステップを標準化するために自動システムが登場しているが、これらの「予備洗浄」装置に関する具体的な性能基準は存在せず、評価が困難である。
目的
既存の ISO ベンチマークに基づいて、自動内視鏡洗浄装置の標準化された評価方法を提案し、市販されている、化学薬品を使用しないブラシレスシステムである AquaTYPHOON 装置の洗浄効果を評価する。
方法
ISO 15883-4:2018 および ISO 15883-5:2021 のプロトコールを用いて、AquaTYPHOON を試験した。洗浄効果は、(i)緑膿菌(Pseudomonas aeruginosa)バイオフィルムを有する代理機器、(ii)血液で汚染された複雑な代理機器、(iii)Austrian test soil で汚染された内視鏡で評価した。洗浄後に残留蛋白質および全有機体炭素(TOC)量を測定し、処置基準値(蛋白質 6.4 μg/cm2、TOC 12 μg/cm2)と比較した。
結果
すべての試験条件において、AquaTYPHOON は、1 カ所の代理位置を除いて、処置基準値未満の蛋白質および TOC 量を達成した。装置は汚染物質量の有意な減少を示し、内視鏡の消毒前の洗浄における効果の要件を満たした。
結論
本研究では、ISO に基づく方法を用いた自動予備洗浄装置の評価は実行可能であることが裏付けられ、AquaTYPHOON の効果が確認されている。患者の安全性を向上させ、ヒューマンエラーを減らし、実臨床での内視鏡再処理方法を標準化するためには、そのような装置の明確な性能基準を定めることが不可欠である。
監訳者コメント:
本研究は、内視鏡用手洗浄を自動化する AquaTYPHOON の性能を ISO 15883 基準で評価し、物理的作用のみで複雑なチャンネル内の有機物を基準値以下に除去可能であることを示した。ヒューマンエラーが避けられない用手洗浄に対し、自動化による標準化と再現性の確保が高度消毒の確実性を高める意義は大きい。予備洗浄装置に特化した専用規格は未整備だが、既存の ISO 枠組みを適用した本検証は、今後の臨床運用と安全性評価の重要な指針となる。自動洗浄の導入は、スタッフの作業負担軽減と患者安全を両立させるための不可欠なステップである。
