Urinary tract infection is associated with 2.4-fold increased risk of surgical site infection in hip fracture surgery: systematic review and meta-analysis

背景

大腿骨近位部骨折患者において、尿路感染症（UTI）をスクリーニングまたは治療すべきかどうかに関してコンセンサスはない。

目的

大腿骨近位部骨折患者における周術期 UTI と手術部位感染症（SSI）の関連、およびこれらの患者における尿道カテーテル留置と SSI の関連について評価すること。

方法

周術期 UTI と SSI の関連および／または尿道カテーテル留置と SSI の関連を評価した研究を特定するために、PubMed、Embase、CINAHL、Cochrane Library で検索した。対象とする論文は、用語 UTI を使用または UTI を症候性細菌尿と定義している場合とした。

結果

計 4,139 報の研究が特定され、8 報を対象とした。周術期 UTI と SSI を評価した 7 報のメタアナリシスにおいて、SSI 発生率は UTI 患者 1,217 例の 7.1％（95％信頼区間［CI］3.8 ～ 13.2）であったのに対して、非 UTI 患者 36,514 例では 2.4％（95％CI 1.0 ～ 5.7）であった（OR 2.41、95％CI 1.67 ～ 3.46、P ＜ 0.001）。UTI を症候性細菌尿と明確に定義した 3 報において、SSI 発生率は UTI 患者では 5.7％（95％CI 4.0 ～ 8.1）、これに対して非 UTI 患者では 1.1％（95％CI 0.2 ～ 5.2）であった（OR 3.00、95％CI 0.55 ～ 16.26、P ＝ 0.20）。尿道カテーテル留置と SSI を評価した研究は 1 報であった。

結論

大腿骨近位部骨折患者の周術期 UTI は SSI の高リスクと関連するが、UTI の定義の異質性によりエビデンスは限定的である。大腿骨近位部骨折患者において、周術期 UTI の可能性を考慮し、SSI 発生率を低減するために必要に応じて治療を実施することを提言する。
