医療施設における患者管理に対する呼吸器ウイルス感染の迅速スクリーニングの影響★★

背景
医療施設において、急性ウイルス性呼吸器感染症は、治療室の混乱や、患者の病的状態や死亡の主因の 1 つである。迅速な診断が患者管理を改善する可能性がある。
目的
入院患者の管理に対する呼吸器ウイルスの迅速分子スクリーニングの影響を評価すること。
方法
University Hospital of Poitiers で実施された 4 年間の後向き観察研究中に、症状のある成人患者は、救急部入室時に検体を採取され、呼吸器ウイルスについて検査された。結果は、迅速マルチプレックス PCR（R-PCR 群、Biofire® FilmArray Respiratory panel 2［Biomerieux、フランス］）では入院前に、従来のマルチプレックス PCR（C-PCR 群、Respiratory Multi Well System MWS R-GENE®［Biomerieux、フランス］）では入院中に入手できた。結果までの時間、救急部への滞在期間と入院期間、個室／二人部屋への入院、入院後最初の 5 日間における患者の部屋の変更、救急部入室時における抗菌薬の投与と再評価、感染による転帰を比較した。
結果
全体で、132 例の入院患者が C-PCR 群に、496 例の入院患者が R-PCR 群に組み入れられた。結果までの時間（3.1 ± 2.7 時間対 22.2 ± 14.2 時間［P ＜ 0.001］）、救急部入室時における抗菌薬の投与（66.1％ 対 80.3％［P ＜ 0.001］）、入院後の患者の部屋の変更（0.49 ± 0.57 対 0.63 ± 0.76［P ＜ 0.05］）は、R-PCR 群の方が有意に少なかった。
結論
この研究で、入院時のマルチプレックス PCR による呼吸器ウイルスのリアルタイムスクリーニングは、医療従事者が患者の流れを管理するのに極めて重要な助けになることが、確認された。
監訳者コメント：
AS および IC のマネジメントにおいて、入院時の呼吸器ウイルス遺伝子検査が有用であった。
