Vancomycin-resistant enterococcus: emergence to endemicity in a tertiary hospital in Singapore

背景

国内および国際的な推奨と整合して、Singapore General Hospital におけるバンコマイシン耐性腸球菌(vancomycin-resistant enterococci;VRE)サーベイランスは、2020 年 8 月から規模が縮小されて免疫不全患者に限定された。

目的

シンガポールの 3 次病院における、2018 年から 2023 年までの間の VRE の疫学の進化および VRE のリスク関連因子を記述すること。

方法

2018 年 1 月から 2023 年 12 月の間に入院した患者を対象とした。VRE サーベイランスの基準は、2020 年 8 月と 2022 年 11 月に変更された。すべての VRE(VRE[全体])、サーベイランスにより検出された VRE(VRE[サーベイランス])、VRE 臨床分離株(VRE[臨床])、VRE 医療関連感染(医療関連 VRE[感染])、VRE 菌血症(VRE[菌血症])の発生率についてレビューした。3 つの症例対照研究を実施した。環境スクリーニングを行った。

結果

6 年間の研究期間中、5,173 例で VRE が検出され、そのうち 3,141 例(60.7%)は医療関連 VRE を有し、5,157 例(99.7%)の分離株はエンテロコッカス・フェシウム(Enterococcus faecium)で、4,336 例(84%)の分離株は vanA を保有していた。121 例(2.2%)は VRE 菌血症を有し、死亡率が 50.4%(論文本体では48.8%ですが、アブストラクト原文に沿って訳しております)であった。VRE(全体)、VRE(サーベイランス)および VRE(臨床)については、発生率に有意な変化が認められたが、医療関連 VRE (感染)と VRE(菌血症)は安定していた。VRE 獲得は、メチシリン耐性黄色ブドウ球菌(meticillin-resistant Staphylococcus aureus;MRSA)との同時感染または保菌(オッズ比 9.4、95%信頼区間 1.3 ~ 66.8、P < 0.02)、カルバペネマーゼ産生腸内細菌目細菌(carbapenemase-producing Enterobacterales;CPE)との同時感染または保菌(7.2、2.0 ~ 26.0、P < 0.001)、集中治療室(ICU)入室(6.1、2.8 ~ 13.2、P < 0.001)、血液透析(4.6、1.8 ~ 12.0、P < 0.001)、ならびに先立つ 3 か月間における手術(3.7、1.6 ~ 8.3、P < 0.001)、バンコマイシンの使用(28.2、5.4 ~ 146.5、P < 0.001)、およびメトロニダゾールの使用(4.4、1.0 ~ 19.0、P = 0.04)と関連した。VRE 感染についても同様のリスク関連因子が認められた。環境検体全体の 12.5%がVRE 陽性であった。

結論

Singapore General Hospital における VRE の地域的流行状態は、患者および環境の大きなVRE 負荷と関連する。VRE 獲得および感染は、MRSA または CPE との同時感染または保菌、バンコマイシンおよびメトロニダゾールの使用、ICU 入室および手術歴と関連していた。標的を絞った感染予防および抗菌薬適正使用支援は、VRE(感染)を減少させる可能性がある。

監訳者コメント

シンガポールの病院における VRE 伝播と医療環境の関連も示唆された。ひとたび VRE 菌血症を発症すれば、その死亡率は非常に高い。日本の病院においても VRE 感染対策の参考になると思われる。
