背景

病院のシンクは、バイオフィルムを形成し、医療関連感染(HAI)を引き起こす多剤耐性病原体(MDRO)のリザーバーである。塩素消毒や排水トラップの交換のような従来の軽減法は、バイオフィルムの再生が持続するために、役に立たないことが多い。

目的

生体電気消毒の試作品である Bioshield Pro に、バイオフィルムに対する塩素の有効性を増強する能力や、汚染された排水トラップからの微生物の広がりを減少させる能力があるかを明らかにするために、実験室での試験や、小児科診療所の実臨床での試験で評価した。

方法

in vitro での試験では、HAI のよく見られるしぶとい微生物であるエンテロコッカス・フェカーリス(Enterococcus faecalis)に対する Bioshield Pro の有効性を、単独の場合と、百万分率(ppm)で1,000 の塩素との併用の場合で評価した。3 か月間の現場での試験も実施した。シンクのボトルトラップでのバイオフィルムの形成は、クリスタルバイオレット染色を使って評価した。細菌のはねかえりは、排水口カバーの近くに置いた寒天平板のコロニー数を使って定量した。これは、水滴からの手の汚染をシミュレーションしている。

結果

実験室での試験では、1.0 V を加えることで、E. faecalis が対照の 39.9%まで減少した(P ≦ 0.001)ことが示された。Bioshield Pro は、塩素による消毒も増強し、細菌の存在を 63.9%から 29.9%まで低下させた(P ≦ 0.01)。臨床現場で、Bioshield Pro は、バイオフィルムを対照の12.1%まで減少させた(P ≦ 0.0001)。消毒薬と組み合わせることで、利益が追加されることはなかった。それに加えて、Bioshield Pro は、はねかえった水の細菌量を、80.0%超減少させた(対照のシンクの 17.67 コロニー形成単位[cfu]/平板から 2.67 cfu/平板まで)。これは、塩素処理のみ(13.67 cfu/平板)より優れていた。このシステムは、低電圧で連続して作動し、試験中の有害事象は報告されなかった。

結論

これらの予備的な試験は、Bioshield Pro が医療施設のシンクの従来の消毒に代わるものとして有望であることを示している。より広範囲で実施することを支持するためには、多様な臨床環境で更に妥当性を確認する必要がある。
