変化する促進要因:COVID-19 パンデミックからパンデミック後の時代の歯科医院における手指衛生遵守率の縦断的分析★

背景

手指衛生は感染症の予防にとって、特に唾液、血液およびエアロゾルへの曝露量の多い歯科において極めて重要である。COVID-19 パンデミックにより一時的に手指衛生実践が改善した一方、パンデミックの様々なフェーズにわたる歯科医院における手指衛生遵守率の長期的な変化と主要な促進因子については、十分に理解されていない。

方法

中国の 3 次歯科病院 1 施設において縦断的研究(2020 年 1 月から 2025 年 6 月)を実施して、世界保健機関(WHO)の「手指衛生の 5 つの瞬間」のフレームワークを用いて手指衛生遵守率の評価を行った。外生変数付き自己回帰和分移動平均(Autoregressive Integrated Moving Average with Exogenous Variables:ARIMAX)モデルにより、主要な促進因子(設備の十分さ、知識の保有率、微生物モニタリングにおける合格率、一人当たり消費、および患者の来院数)を特定し、パンデミックの期間とパンデミック後の期間の間におけるそれらの影響の変化を調べた。

結果

全体的な手指衛生遵守率は、84.6%から 89.0%にまで増加した。ARIMAX モデルから、パンデミック中には設備の十分さが最も強い予測因子であったが(β = 0.74、P < 0.001)、その影響はパンデミック後に減少した(交互作用のβ = -0.58、P = 0.001)ことが明らかになった。知識の保有率とモニタリング合格率は、一定して陽性の予測因子であり続け、パンデミック後に知識の重要性が増大した。看護師では最も高い手指衛生遵守率(> 94%)が示された一方、研修医および清掃スタッフでは遵守率が最も低かった。遵守率は、「体液に曝露された後」が最も高く、「患者に触れる前」が最も低かった。

結論

歯科病院における手指衛生遵守の促進因子は、変化が大きく、フェーズによって変わる。パンデミック期間中はインフラが極めて重要であったが、パンデミック後は知識と実践の質が大きく重要性を増した。持続的な改善をするには、インフラへの投資から手指衛生文化の醸成への転換とともに、遵守率が低いスタッフと瞬間に対する焦点を絞った戦略が必要である。

監訳者コメント

本研究は、歯科病院における手指衛生遵守率が COVID-19 流行期に上昇し、その後低下したものの一定の改善は維持されたことを示した。また、遵守行動を規定する要因が、パンデミック期の「感染への恐怖や監視」から、ポストパンデミック期の「職場環境や組織文化」へと変化したことを明らかにした。手指衛生を持続的に向上させるためには、危機意識に依存するのではなく、組織文化として定着させる取り組みが重要であることを示唆する研究である。
