背景

消化管内視鏡の再処理の適切性を確保するために、定期的な微生物学的サーベイランスが推奨されている。

目的

内視鏡の種類に応じて汚染に差があるかどうかを検討した。

方法

スイスの 3 次病院で消化管内視鏡から採取された微生物学的サーベイランス検体合計 437 検体を分析した。Standard SN EN ISO 15883-4:2018 に従った洗浄消毒器における再処理の後に、検体採取を行った。微生物学的分析では、内視鏡洗浄液をろ過し、羊血液寒天培地に移した。これらのプレートを 37°C でインキュベートした。細菌の増殖が検出された場合は、マトリックス支援レーザー脱離イオン化飛行時間型質量分析法またはその他の微生物学的同定法を用いて菌種を同定し、コロニー形成単位(cfu)を計数した。汚染は、濃度 20 cfu/20 mL 以上の微生物が検出されること、または、cfu 数を問わず腸内細菌目細菌、腸球菌(Enterococcus)属菌、緑膿菌(Pseudomonas aeruginosa)およびその他の非発酵菌、緑色連鎖球菌(Streptococcus viridans)、黄色ブドウ球菌(Staphylococcus aureus)が検出されることと定義した。

結果

全体で、胃内視鏡 28 本から 204 検体、大腸内視鏡 23 本から 146 検体、超音波内視鏡 5本から 45 検体、十二指腸内視鏡 4 本から 27 検体、小腸内視鏡 3 本から 15 検体が収集された。内視鏡 5 本から収集された 11 検体(2.5%)で汚染が認められた。汚染を示す検体の割合は内視鏡の種類によって大幅に異なり、超音波内視鏡由来検体の 20.0%(9/45 検体)、大腸内視鏡由来検体の 1.4%(2/146 検体)、胃内視鏡由来検体の 0.5%(1/204 検体)、十二指腸内視鏡由来検体の 0%(0/27 検体)、小腸内視鏡由来検体の 0%(0/15 検体)に汚染が認められた(P < 0.001)。

結論

全体として、汚染された内視鏡の割合は小さかった。内視鏡の種類による有意な差が認められ、超音波内視鏡は最も成績が悪かった。

監訳者コメント:

内視鏡器具も構造により、特に超音波内視鏡器具が Standard SN EN ISO 15883-4:2018 に従った洗浄消毒器における再処理の後に汚染されやすいことがこの論文からわかる。器具の改良ならびに再生処理の仕方など、課題があることがわかる。
