Hygiene issues in urinal bottle use in Danish nursing homes without bedpan washers: a qualitative interview study with healthcare assistants and assistant nurses caring for residents with and without catheters

背景

30 年前、デンマークのナーシングホームで、より自宅のような環境を作り出すための変化が始まり、介護施設の施設らしさをなくすためにおまるを除去した。このことによって、汚物処理室を含む付属室が閉鎖された。しかし、適切な洗浄設備なしで尿瓶の使用を継続することによって、衛生上の懸念や健康のリスクが増加した。自宅のような雰囲気と不十分な衛生設備の間にある不釣り合いが、安全な介護を提供するための課題となっている。

目的

この研究は、尿瓶の取り扱いに関するナーシングホーム職員の実践、課題、認識を探ることが目的であった。

方法

尿瓶に対する日常の取り扱いを理解するために、ナーシングホーム職員の質的で半構造的な個人面接を採用した。参加者（N ＝ 8）は、デンマークの地方自治体の 1 つにある 4 つのナーシングホームから採用した。

結果

結果から、職員が様々な洗浄方法を使っていたことと、特異的なプロトコールからの明確な指針がなかったことが明らかになった。その代わりに、洗浄方法は、個人の決まったやり方に頼っていることが多かった。保護具は、職員、入居者、同僚を病原体から保護するのに不可欠である。しかし、職員は、様々な状況に対して適切な装備を決めるのが難しいことを報告した。実践の多くは、適切な仕事の流れについての個人の考えに基づいていたので、明確なガイドラインが必要であることをはっきりと示している。

結論

デンマークのナーシングホームでは、標準化された手順や適切な洗浄設備がないことによって、尿瓶の取り扱いが非衛生的になっている。職員は、これらのことが、入居者の尿路感染や、入居者が病気に罹りやすいことと明らかにつながっているにもかかわらず、感染のリスクであると認識できていないことが多い。この研究は、全国的なガイドライン、設備の改善、安全で持続可能でエビデンスに基づいた洗浄の実践に向かう変化が緊急に必要とされていることをはっきりと示している。
