背景

COVID-19 パンデミックの間、医療従事者の個人防護具として長袖ガウンが推奨されていた。ガウン使用の目的は、患者やその環境との接触時における、ユニフォームや腕を介した感染性微生物の伝播を予防することである。しかし、ガウンの使用には、コスト、医療従事者の手間、および生じる廃棄物といったかなりの負担が伴う。

目的

COVID-19 やその他の呼吸器ウイルス感染症の患者のケアを行う際の、院内伝播を予防するためのガウンの使用に関する現在の知識について評価すること。

方法

PRISMA ガイドラインを用いて、5 つのデータベース(Medline、Embase、Web of Science、Cochrane、Google Scholar)の検索を 2023 年 4 月 11 日までを対象に行った。

結果

検索により、関連する可能性のある研究 2,667 件が同定され、うち 30 件を選択し、4 つのカテゴリーに分けた。研究 12 件において、ガウンの汚染率は 0% から 77.5%(中央値 1.43%)の範囲であった。研究 7 件中 3 件では、ウイルスの感染性の残存期間が Tyvek製 カバーオールおよび プラスチック製ガウンで最も長く、綿製およびポリエステル製ガウンで最も短かった。研究 7 件中 2 件では、医療従事者の防護具と感染との間に保護作用が認められた。そして、研究 4 件中 3 件では、半袖、綿製ガウン、またはガウン無着用の場合に、標準的ガウンの場合と同レベルの保護効果があると結論されていた。

結論

ウイルス RNA は衣服上に検出されることがあるが、ウイルスが医療従事者および/または患者に伝播するかどうかは明らかでない。長袖ガウンがその他の衣服より保護効果が高いとするエビデンスは、まだ不十分である。したがって、適切な対照を設定し、十分な検出力を有する、伝播について現実の条件をシミュレーションした実験室での実験が必要である。

監訳者コメント

呼吸器ウイルス伝播予防を目的としたガウンの保護効果についてのシステマティックレビュー。ガイドラインなどで推奨されている対策が十分なエビデンスのもと行われているのか、不十分なものなのかを理解し、立ち位置を確認しながら対策を行うことは重要である。
