From ward to intensive care unit: clinical drivers of adverse outcomes in Clostridioides difficile infection — A retrospective cohort study

背景/目的

クロストリジオイデス・ディフィシル(Clostridioides difficile)感染症(CDI)は、依然として、院内下痢症の主な原因の 1 つであり、集中治療室(ICU)や中間ケアの患者は、有病率と死亡率が特に高い。予後モデルは存在するが、予測の正確さが依然として限られている。この研究は、三次ケアの環境における CDI の有害な転帰、再発、長引いた入院の決定因子を特定することを目的とした。

方法

入院した CDI の症例 87 例を後向きに解析した(2012 年から 2014年)。重症の CDI は、検査と臨床の基準によって定義した。主要エンドポイントは、CDI の主要有害臨床事象(手術、院内死亡、または重症で複雑な経過)とした。副次的転帰は、再発、入院期間、等とした。ロジスティックおよび線形回帰分析をブートストラップ検証(1,000 回の反復)とともに実施した。

結果

87 例の患者(平均年齢が60.7 ± 15.2 歳、48%が 65 歳以上、53%が免疫抑制)のうち、37 例(42.5%)が ICU への入室/中間ケアへの入院を必要とした。CDI の主要有害臨床事象は 16.1%に発生し、ICU への入室と関連していた(オッズ比[OR]、4.26、95%信頼区間[CI]、1.29 ~ 16.76、P = 0.017)。院内死亡(13.8%)は、ICU への入室(OR、8.89、95%CI、2.15 ~ 60.65、P = 0.0017)および年齢が 55 歳以上であることと関係があった。重炭酸ナトリウム療法は、転帰が不良であることを予測した(OR、2.9、P = 0.028)。CDI の再発は 21.8%で起こった。バンコマイシン耐性腸球菌(VRE)の定着は、ICU/中間ケアの患者において 16 倍を超えるリスクの増加をもたらした(OR、16.20、P = 0.015)。非経口栄養は、一貫して、重症の CDI(OR、2.49、P = 0.041)および長引いた入院と関連していた。また腹水と関連していた(P < 0.01)。ピペラシリン/タゾバクタムやセファロスポリンへの曝露は、院内/重症の CDI のリスクを増加させた一方で、トリメトプリム/スルファメトキサゾールはそれよりもリスクが低い選択肢であるようにみえた。

結論

高齢、ICU への入室、腹水、VRE の定着、重炭酸ナトリウム療法、非経口栄養が CDI の有害な転帰と相関していた。これらの因子は、高リスク患者における適正使用支援/管理戦略に情報をもたらす可能性がある。
