背景

水回り設備が多剤耐性病原菌の病院アウトブレイクに関与する可能性がある。

目的

Verona integron-encoded metallo-ß-lactamase type 2 産生カルバペネム耐性緑膿菌(Pseudomonas aeruginosa)(CRPA-VIM)のアウトブレイクを記録し、感染制御策を評価すること。また、オランダの医療機関における多剤耐性病原菌が係わる水系アウトブレイクを概観すること。

方法

疫学的解析、選択培養、PCR、および全ゲノムシークエンシング(WGS)によりアウトブレイクの感染源を特定した。全国サーベイランスデータベースを調べた。

結果

2023 年 12 月、ICU 患者 3 例が、複数部位塩基配列型(MLST)ST111 の CRPA-VIM 陽性を示した。汚染されたシンクが感染源と特定された。対策を実施したにもかかわらず、2024 年 3 月から 4 月に新たなクラスターとして CRPA-VIM 陽性例が 5 例発生した。WGS により、このクラスターと使用停止したシンク(2021 年 11 月)および患者 2 例(2021 年 12 月および2023 年 4 月)との関連が認められた。接触追跡と感染源調査により、患者間伝播はないことが示され、シンクが単独感染源として特定された。接触予防策、清掃および衛生手順の強化、再教育、飛散やエアロゾルを低減する給水口の導入および頻繁な交換、蛇口の形状変更などの対策を実施したが、伝播を阻止することはできなかった。ICU の病室の水回り設備をすべて取り除いた後は、新規症例が発生しなかった。アウトブレイク株はこの病院に固有ものであり、全国サーベイランスにおいて遺伝的クラスター形成は認められなかった。全国で、1 年に 3 ~ 5 件の高度耐性微生物による水系アウトブレイクが報告されている。

結論

CRPA-VIM は、シンクから ICU 患者に伝播し、患者間では伝播しなかった。感染制御には患者および環境の包括的サーベイランスが必要である。今回のアウトブレイクは、環境の水使用を制限し、排水をなくすことにより終息した。今後病院は、院内感染リスクを低減するため、排水やシンクからの汚染を最小化するよう設計すべきである。

監訳者コメント

本事例においてもシンクがカルバペネム耐性緑膿菌のリザーバーであった。ICU の病室の水回り設備をすべて取り除くことで、ようやく終息したことから、水回りの感染対策の重要性を示している。
