Prognosis analysis and infection-related risk factors of multi-drug-resistant bacteria isolated from a general hospital in China, 2019 - 2023

背景

多剤耐性細菌に関する検出動向、感染予後、ならびに院内感染症のリスク因子の分析は、感染制御の取り組みを改善する上で有用となり得る。

方法

2019 年から 2023 年にわたる中国の単一病院の入院患者のデータを用いて、カルバペネム耐性肺炎桿菌（Klebsiella pneumoniae）（CRKP）、カルバペネム耐性アシネトバクター・バウマニー（Acinetobacter baumannii）（CRAB）、カルバペネム耐性緑膿菌（Pseudomonas aeruginosa）（CRPA）、およびメチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus；MRSA）の検出率における動向を解析した。Kaplane-Meier 法を用いて、肺炎桿菌、A. baumannii、緑膿菌、および黄色ブドウ球菌の耐性および感受性株に感染した患者の生存予後を評価した。1:1 のマッチド症例対照研究を用いて、リスク因子の分析を行った。

結果

χ² 傾向検定から、CRKP検出の増加率に統計学的有意差が認められた（χ² ＝ 25.481、P ＜ 0.001）。CRKP および CRAB は主として集中治療室および手術時に検出された。CRPA および MRSA は主として内科病棟で検出された。多剤耐性菌院内感染症の主な感染部位は呼吸器系および血液系であった。多重ロジスティック回帰分析では、患者における腫瘍（オッズ比［OR］＝ 2.886）、コルチコステロイド使用（OR ＝ 1.403）、アルブミン値低下（OR ＝ 0.701）、抗菌薬併用投与（OR ＝ 2.873）、手術（OR ＝ 3.317）、胃管留置（OR ＝ 1.738）、人工呼吸器装着（OR ＝ 1.616）、ならびに中心静脈カテーテル留置（OR ＝ 1.778）は多剤耐性菌院内感染症の独立リスク変数であることが示された（P ＜ 0.05）。短期間の抗菌薬使用（OR ＝ 0.665）は多剤耐性菌院内感染症に対する保護因子であった（P ＜ 0.05）。

結論

CRKP 感染症の高い致死率は無視すべきでない。臨床的取り組みでは、集中治療室における下気道および血液系の CRKP 感染症に焦点を置くべきであり、抗菌薬の適正な使用に対する管理を強化すべきである。
