心臓手術における抗菌薬予防投与のガイドラインの比較分析：チェコとスロバキアでの調査★

背景
心臓手術における手術部位感染症（SSI）は、患者の転帰に重大な影響を与える。この研究は、SSI の発生率を減少させることにとって極めて重要である、心臓での抗菌薬予防投与のチェコとスロバキアの病院におけるガイドラインについて検討した。
方法
両国の心臓外科部門で、外科的抗菌薬予防投与（SAP）プロトコール、抗菌薬の選択、用量、タイミング、術後感染率に焦点を当てた電子調査を実施した。回答を 3 か月にわたって収集し、網羅的な概観が確保できるようにした。
結果
調査した病院の大部分（79％）は、SAP を実施しているが、ガイドラインでは、ばらつきが目立つ。予防投与の第一選択としては、セファゾリンが多いが、投与レジメンは様々である。タイミングが一致していないこと、予防投与期間が長いこと、などが、特にインプラントに関係する手技で課題である。過去 1 年以内に、ガイドラインの定期的な見直しを報告しているのは病院の 18％であり、最新の実践が必要であることをはっきりと示している。
結論
この研究は、SAP の実践を標準化し、変化するエビデンスに適合させ、ガイドラインの定期的な見直しを実施することの重要性を示している。観察されたばらつきは、心臓外科における SSI の予防戦略を強化し、究極的には、患者の転帰の改善に貢献する機会をはっきりと示している。
監訳者コメント ：
チェコとスロバキア両国での心臓外手術における外科的抗菌薬予防投与（SAP）プロトコールの遵守状況を国レベルで調査した報告。その結果、SAP プロトコールは約 80％の病院で実施されているが、病院ごとに実践のばらつきが多かった。
