迅速な標的 UVC 除染システムの試験管内および実使用環境での有効性★★

背景
紫外線C(UVC)除染システムは、医療現場において手動での清掃を補完する目的で使用される。これらのシステムは通常、固定された垂直型のランプを使用して部屋全体をターゲットにしている。
目的
迅速で標的を絞った病室の除染のために設計された、多関節アームにエミッターを搭載した新規装置の試験管内および使用中の有効性を評価する。
方法
メチシリン耐性黄色ブドウ球菌(MRSA、約106 コロニー形成単位(cfu)、肺炎桿菌(Klebsiella pneumoniae、約 106 cfu)、およびクロストリジオイデス・ディフィシル(Clostridioides difficile、約 105 cfu)の芽胞を、低度の汚染(0.3% BSA)および高度の汚染(10% BSA/人工糞便)の条件でステンレス製生物学的インジケーター(BI)に接種した。UVC 除染後にこれらの BI から細菌を回収し、菌数を測定しコントロールと比較した。また、実際の使用環境での効果を、接触プレートを用いた好気性菌コロニー数(ACC)のサンプリングによって評価した。C. difficile についても同様にサンプリングを行った。サンプルは清掃前、用手清掃後、UVC 除染後に採取した。
結果
MRSA および K. pneumoniae では、低度および高度の汚染条件下で、それぞれ 2.97 ~ 4.87 log10 cfuおよび 0.53 ~ 3.63 log10 cfuの減少が確認された。C. difficile に対する効果は、低度汚染の 1 箇所でのみ認められ(1.12 log10)、人工糞便では効果が見られなかった。ACC は、ベッドの足元のレール(75 cfu/25 cm²)およびトイレのフラッシュ部分(67 cfu/25 cm²)で最も高い値を示した。用手清掃後も 50%の表面に細菌が残存し、UVC 除染後には 30%に減少した。C. difficile は 1 箇所で残存していた。
結論
本システムは、特定の対象を絞った除染に効果的であり、高頻度接触面や医療機器に使用できる可能性がある。サイクル時間が短いため手術室や麻酔室のようにターンアラウンドタイムが短い場所でも運用が可能である。
監訳者コメント:
この研究では、従来のシステムと比較して可動式のアームを使用して、短時間で特定の表面や機器を効果的に除染する新しいシステムである。全室の除染や C. difficile 汚染が疑われる場合は、サイクルタイムの長い他のシステムを検討すべきである。
