Povidone-iodine and silver nitrate are equally effective in eradicating staphylococcal biofilm grown on a titanium surface: an in-vitro analysis

背景

デブリードマンと抗菌化学療法によるインプラント温存（DAIR）手術を実施する際に使用すべき洗浄液および濃度についてのコンセンサスはない。

目的

5 つの抗菌溶液について、バイオフィルムの最小根絶濃度を測定し、チタン表面に埋め込まれたブドウ球菌のバイオフィルムの根絶における有効性を比較すること。

方法

メチシリン感性黄色ブドウ球菌（Staphylococcus aureus）（MSSA）とメチシリン耐性黄色ブドウ球菌（MRSA）の ATCC 標準株を、多孔性の Ti–6Al–4V 寛骨臼スクリューキャップ上で増殖させた。抗菌溶液は、ポビドンヨード、リファンピシン、硝酸銀、硫酸銅、クロルヘキシジンとした。バイオフィルムの最小根絶濃度を MSSA と MRSA について算出した。24 時間後、スクリューキャップを3分間各溶液に曝露させた。各検体からの細菌の分離は、ボルテックスによる攪拌とトリプリケートの寒天平板上での footprint によって実施した。コロニー形成単位（cfu）を攪拌の前と後に数え、cfu/mL のデルタを、各溶液について算出した。cfu の log で 3 倍の減少を、溶液の有効性の尺度と見なした。群間の比較はフィッシャーの検定で行った。

結果

MSSA と MRSA に対するバイオフィルムの最小根絶濃度は、それぞれ、次のようであった。ポビドンヨードが 8,000μg/mL と 16,000μg/mL、リファンピシンが 64μg/mL と 128μg/mL、硝酸銀が 10,000μg/mL と 5,120μg/mL、硫酸銅が900μg/mL と 900μg/mL、クロルヘキシジンが 16μg/mL と 32μg/mL。リファンピシン、硫酸銅、 クロルヘキシジンは、cfu 減少のデルタが 8 log を超えていたポビドンヨード（P ＜ 0.01）や硝酸銀（P ＝ 0.015）と比べると、MSSA や MRSA のバイオフィルムに対して効果がなかった。ポビドンヨードと硝酸銀は、目に見える MSSA（P ＝ 0.005）や MRSA（P ＝ 0.014）がなく、footprint は陰性であった。

結論

ポビドンヨードと硝酸銀のみが、24 時間のバイオフィルムを少なくとも 99.9％根絶できる洗浄液であった。
