背景
タウロリジン・クエン酸(・ヘパリン)ロック溶液は、中心ライン関連血流感染症(CLABSI)予防のための有望かつ安全な方法であることが示唆されている。
目的
小児腫瘍患者の CLABSI 予防におけるタウロリジン・クエン酸(・ヘパリン)ロック溶液の有効性を検討すること。
方法
2020 年から 2023 年にかけて、オランダのPrincess Máxima Centre for paediatric oncology において、評価者を盲検化した無作為化対照試験を実施した。トンネル型中心静脈アクセスデバイス留置を受けている小児腫瘍患者を本試験に適格とした。患者計 462 例を対象に、タウロリジン・クエン酸(・ヘパリン)ロック溶液をヘパリンロック溶液と比較することとした。患者の追跡調査を、中心静脈アクセスデバイス挿入後 90 日間行った。主要転帰は、中心静脈アクセスデバイス挿入後から追跡調査終了時までの CLABSI 初発の割合とした。Intention-to-treat 解析および per-protocol 解析を実施した。
結果
合計で、232 例をヘパリンロック溶液群に、231 例をタウロリジン・クエン酸(・ヘパリン)ロック溶液群に割り付けた。CLABSI が計 47 件観察された。Intention-to-treat 解析から、CLABSI が観察されたのは、ヘパリンロック溶液群では 26 例(11.2%)であったのに対して、タウロリジン・クエン酸(・ヘパリン)ロック溶液群では 21 例(9.1%)であり、発生率比(IRR)は 0.81(95%信頼区間[CI]0.46 ~ 1.45)と、タウロリジン・クエン酸(・ヘパリン)ロック溶液群のほうが優れていることが示された。Per-protocol 解析では、CLABSIが観察されたのはヘパリンロック溶液群では 10 例(7.9%)であったのに対して、タウロリジン・クエン酸(・ヘパリン)ロック溶液群では 6 例(4.8%)であり、IRR は 0.59(95%CI 0.21 ~ 1.62)と、タウロリジン・クエン酸(・ヘパリン)ロック溶液群のほうが優れていることが示された。有害事象は、タウロリジン・クエン酸(・ヘパリン)ロック溶液のほうが多かったが、報告はまれであった。
結論
タウロリジン・クエン酸(・ヘパリン)ロック溶液とヘパリンロック溶液 の間で、小児腫瘍患者の CLABSI 発生率に差は認められなかった。
監訳者コメント:
タウロリジン・クエン酸溶液は、抗菌・抗凝固・抗バイオフィルム形成の 3 つの効果をもち、血管留置カテーテル関連菌血症の予防に効果があることがいくつかの論文で示されており、海外では市販されている。特に中心静脈カテーテルが挿入された維持透析患者における CLABSI 予防においての報告が多いようである。今回は小児腫瘍患者での CLABSI に対する RCT を実施し有意差はなかったものの、傾向はみられており大規模研究が待たれる。
