Trends in the hand hygiene practices using alcohol-based hand rubs in Japanese hospitals before and after the novel coronavirus pandemic: an observational study using national surveillance data

背景

医療関連感染（HAI）は、世界中で、医療施設における懸念事項となっていて、擦式アルコール製剤（ABHR）を使った手指衛生が、その予防において、本質的に極めて重要である。これまでの研究では、COVID-19 パンデミック中、手指衛生の遵守が向上したことが報告されているが、日本の医療環境における真の状況は、依然として、はっきりしていない。

方法

この観察研究では、病院における COVID-19 パンデミックの前後の ABHR の使用に焦点を当てて、日本の全国調査からデータを検索した。感染対策向上加算 1 を受け取れる認定施設からデータを抽出した。研究は、5 年に亘り（2019 から 2023 年）、四半期に分割され、年間変化率を評価するために Joinpoint 回帰分析が採用された。

結果

全体的には、患者 1 人当たり 1 日当たりの ABHR の使用が、パンデミック中、重症管理室と一般病棟の両方で有意に増加した。しかし、重症管理室の年間変化率は、2021 年の Q4 から 2023 年の Q1 まで下向きの傾向を示し、一般病棟での ABHR の使用は、WHO の推奨量より下のままであった。

結論

この傾向分析では、COVID-19 の前後の期間を比較することによって、日本の病院における ABHR の使用の最近のパターンがはっきりと示された。ABHR の使用の増加が時間とともに観察されたが、手指衛生の遵守を促進する持続した取り組みが、特に一般病棟で必要である。
