日本の病院における擦式アルコール製剤を使った手指衛生のやり方の新型コロナウイルスパンデミック前後の傾向：全国調査データを使った観察研究★★

背景
医療関連感染（HAI）は、世界中で、医療施設における懸念事項となっていて、擦式アルコール製剤（ABHR）を使った手指衛生が、その予防において、本質的に極めて重要である。これまでの研究では、COVID-19 パンデミック中、手指衛生の遵守が向上したことが報告されているが、日本の医療環境における真の状況は、依然として、はっきりしていない。
方法
この観察研究では、病院における COVID-19 パンデミックの前後の ABHR の使用に焦点を当てて、日本の全国調査からデータを検索した。感染対策向上加算 1 を受け取れる認定施設からデータを抽出した。研究は、5 年に亘り（2019 から 2023 年）、四半期に分割され、年間変化率を評価するために Joinpoint 回帰分析が採用された。
結果
全体的には、患者 1 人当たり 1 日当たりの ABHR の使用が、パンデミック中、重症管理室と一般病棟の両方で有意に増加した。しかし、重症管理室の年間変化率は、2021 年の Q4 から 2023 年の Q1 まで下向きの傾向を示し、一般病棟での ABHR の使用は、WHO の推奨量より下のままであった。
結論
この傾向分析では、COVID-19 の前後の期間を比較することによって、日本の病院における ABHR の使用の最近のパターンがはっきりと示された。ABHR の使用の増加が時間とともに観察されたが、手指衛生の遵守を促進する持続した取り組みが、特に一般病棟で必要である。
