背景
擦式アルコール製剤による手指衛生(HH)の前後に、ジェルネイルを施した(gel polished:GP)爪、標準的なマニキュアを塗布した(standard polished:SP)爪および未塗布の(unpolished:UP)爪における細菌負荷を評価した。
方法
医療従事者 46 名の両手の GP 爪 3 本、SP 爪 2 本および UP 爪 5 本を分析した。研究中は爪の長さを 2 mm以下に維持した。塗布後 1、4、7、14 および 21 日目の HH 前後に、爪表面の培養を血液寒天プレートで行った。ネイル剤群、参加者、手の左右、指、評価日、ネイル剤群と評価日の交互作用を独立因子として、ポアソン回帰法を用いて爪の細菌数を推定した。
結果
全体で 460 本の爪を評価した。内訳は SP 爪 92 本(20%)、GP 爪 138 本(30%)、UP 爪230 本(50%)であった。HH 前においては、SP 爪は 4日目に GP 爪より細菌負荷が高く、21 日目に GP 爪および UP 爪より細菌負荷が高かった(UP 爪の細菌数は GP 爪より多かった)。HH 後においては、SP 爪は4 日目に UP 爪および GP 爪より細菌負荷が高く、14 日目に UP 爪より細菌負荷が高く、21 日目に UP 爪および GP 爪より細菌負荷が高かった。
結論
UP 爪と比較して、GP 爪において塗布後3週間以内により高い細菌負荷との関連は認められず、さらには 21 日目の HH 前に細菌負荷が有意に低かった。
監訳者コメント:
医療従事者のマニキュアによる爪の装飾は感染管理の観点から伝統的に推奨されていない。本研究では標準的なマニキュアにおいては細菌量の増加から使用は依然として推奨されないが、ジェルネイルについては爪の長さが 2 mm以下の場合は使用の禁止を解除できる可能性がある。本研究だけでは制限があるが、エビデンスに基づいた感染管理の実践として重要な研究である。
