高リスク患者環境における医療用水を対象とした緑膿菌(Pseudomonas aeruginosa)に対する銀ナノ粒子(Ag+)含浸シャワーヘッドの院内有効性試験および抗菌特性評価の必要性

緒言
緑膿菌(Pseudomonas aeruginosa)は、グラム陰性の水系日和見病原体である。細菌のリザーバは脆弱集団に感染する可能性がある。医療においてこのリスクを軽減する方法は、排水口の定期的な微生物学的モニタリングおよび化学的/熱消毒などである。汚染が存続する場合は、バイオバーデンを軽減するために Point-of-use(POU)フィルター/抗菌シャワーが用いられることもある。本研究では、模擬条件下で、銀ナノ粒子(Ag+)を含浸させた抗菌シャワーヘッドの緑膿菌に対する有効性を試験した。
方法
被験(Ag+ 含浸シャワーヘッドから切り取り)および対照(ステンレス鋼)クーポン(約 1 cm2、N = 108)に、10 μL(約 103 コロニー形成単位[CFU])の緑膿菌(NCTC 10662 および野生型)を播種し、カバースリップを乗せた。クーポンを環境温度(相対湿度約 40%)で 0 分、30 分、2 時間、4 時間および 24 時間インキュベートした。クーポンとカバースリップを中和剤溶液に入れた後、ビーズ洗浄を行い、細菌を回収および計数した。
結果
汚染は約 2.90 log10 CFU(±0.14 log10、基準株)から約 3.02 log10 CFU(±0.16 log10、野生型)の範囲であった。緑膿菌のバイオバーデンには、3 種類の菌株のいずれでも有意な減少は認められなかった(P > 0.05)。4 ~ 24 時間に被験クーポンと対照クーポンでは類似した細菌減少が認められ(検出限界未満に低下)、これは乾燥によるものと考えられた。
要約
抗菌シャワーは、経済的および環境的に負担の小さい選択肢ではあるが、医療用水のバイオバーデンを軽減することには適していない可能性がある。これまでの研究では Ag+ の殺菌効果が証明されているが、本研究ではそのような効果は確認されなかった。被験材料における銀濃度は不明であったが、本研究の結果から、抗菌シャワーヘッドは、抗菌活性を示すためにより高濃度の Ag+ を必要とする可能性が示唆される。バイオフィルムに対する有効性を評価するためには、さらなる試験が必要である。
監訳者コメント:
銀ナノ粒子(Ag+)含浸シャワーヘッドが緑膿菌の殺菌効果があるかどうか、検証した論文であるが、結果は芳しくなかった。十分な検証が行われておらず、今後のさらなる検証が必要であろう。
