Outbreak of Mycobacterium mucogenicum bacteraemia due to contaminated water supply in a paediatric haematology-oncology department

6か月間にわたり小児血液・腫瘍内科病棟の患児5例に発生したマイコバクテリウム・ムコゲニカム（Mycobacterium mucogenicum）による血流感染症のアウトブレイクについて報告する。病棟フロアの蛇口から採取した試料により、自動水栓が感染源と疑われることが示唆され、分子的手法により分離株の同一性が確認された。水中の塩素濃度が断続的に低下しており、これが細菌増殖に寄与した可能性がある。感染管理の実施状況を調査したところ、小児の中心静脈カテーテルの出口部が入浴中に適切に覆われておらず、そのため中心静脈カテーテルへの細菌の定着が促進されたことが判明した。汚染された蛇口を交換し、水の至適な塩素処理を行い、中心静脈カテーテルの出口部を不透水性ドレッシングで適切に覆うことにより、アウトブレイクは終息した。本研究により、水中の病原体によるアウトブレイクで調査すべき3つの因子、すなわち感染源、給水経路、および感染管理の実施状況の重要性が明らかになった。

非結核性抗酸菌の一種であるMycobacterium mucogenicumによる感染アウトブレイク事例の紹介である。内視鏡洗浄器や透析設備など、医療環境における非結核性抗酸菌の現況は十分に解明されているわけではない。今後さらなる検討が必要である。
