中国、広州の 3 次病院の泌尿器科における薬剤師主導による多面的な抗菌薬適正使用支援が、抗菌薬使用および臨床転帰に及ぼす影響：分割時系列研究★

背景
泌尿器科における薬剤師主導による抗菌薬適正使用プログラム（ASP）の効果に関する研究には限界がある。
目的
薬剤師主導による多面的 ASP が抗菌薬使用および臨床転帰に及ぼす影響を評価すること。
方法
中国、広州の大規模教育病院の泌尿器科において、1 種類以上の抗菌薬を投与されている入院患者の処方レビューを、2019 年 4 月から 2023 年 3 月まで実施した。薬剤師主導による多面的 ASP 介入は、ガイドライン作成、研修、服薬相談、医療指示の再考、指標のモニタリング、専門家との相談などであった。主要評価項目は抗菌薬使用量とした。分割時系列解析によりデータを分析した。
結果
ASP の実施後には、全体的な抗菌薬使用量、抗菌薬使用率、第 2 世代セファロスポリン系薬、第 3 世代セファロスポリン系薬、フルオロキノロン系薬、および WHO による「Watch」の処方カテゴリーの抗菌薬が速やかに減少した。介入前後の死亡率に差は認められず、分割時系列解析では、入院期間（LOS）への有意な短期効果も長期効果も認められなかった。一方で、抗菌薬の平均費用に有意な短期効果がみられた。
結論
薬剤師主導による多面的 ASP の実施は、LOS、抗菌薬の費用、死亡率を増加させることなく、抗菌薬使用量の低減において確実な効果を示した。
監訳者コメント ：
薬剤師主導の多面的な ASP の実施により、患者の入院期間や死亡率の増加を認めずに、WHO Watch カテゴリーの抗菌薬使用の削減につながった。ASP における薬剤師の役割に益々注目が集まっている。
