Impacts of pharmacist-led multi-faceted antimicrobial stewardship on antibiotic use and clinical outcomes in urology department of a tertiary hospital in Guangzhou, China: an interrupted time-series study

背景

泌尿器科における薬剤師主導による抗菌薬適正使用プログラム（ASP）の効果に関する研究には限界がある。

目的

薬剤師主導による多面的 ASP が抗菌薬使用および臨床転帰に及ぼす影響を評価すること。

方法

中国、広州の大規模教育病院の泌尿器科において、1 種類以上の抗菌薬を投与されている入院患者の処方レビューを、2019 年 4 月から 2023 年 3 月まで実施した。薬剤師主導による多面的 ASP 介入は、ガイドライン作成、研修、服薬相談、医療指示の再考、指標のモニタリング、専門家との相談などであった。主要評価項目は抗菌薬使用量とした。分割時系列解析によりデータを分析した。

結果

ASP の実施後には、全体的な抗菌薬使用量、抗菌薬使用率、第 2 世代セファロスポリン系薬、第 3 世代セファロスポリン系薬、フルオロキノロン系薬、および WHO による「Watch」の処方カテゴリーの抗菌薬が速やかに減少した。介入前後の死亡率に差は認められず、分割時系列解析では、入院期間（LOS）への有意な短期効果も長期効果も認められなかった。一方で、抗菌薬の平均費用に有意な短期効果がみられた。

結論

薬剤師主導による多面的 ASP の実施は、LOS、抗菌薬の費用、死亡率を増加させることなく、抗菌薬使用量の低減において確実な効果を示した。

薬剤師主導の多面的な ASP の実施により、患者の入院期間や死亡率の増加を認めずに、WHO Watch カテゴリーの抗菌薬使用の削減につながった。ASP における薬剤師の役割に益々注目が集まっている。
