抗菌薬の静注から経口への切り替え（IVOS）に関する看護師の姿勢、行動および対応策：イングランド・ミッドランズ地域全域の 2 次ケア病院における看護職員の混合手法調査★

背景
イングランドでは 2 次ケアにおいて静注（IV）抗菌薬が広く使用されている。静注から経口への切り替え（intravenous to oral switch：IVOS）を適時に適切に行うことは、大きな臨床上および業務上のベネフィットをもたらす可能性がある。これまで、IVOS をめぐる抗菌薬適正使用支援（AMS）の取り組みは、抗菌薬の投与を行う看護職員に焦点が当てられておらず、このことが AMS プログラムにおける重大なギャップとなっている。
目的
イングランド・ミッドランズ地域の急性期トラストにおいて、ベッドサイド看護師の組織内の IVOS への関与を評価し、IVOS を改善する方法に関する看護師の意見を報告する。
方法
匿名の自記式混合手法オンライン調査を作成し、2023年 3 月から 5 月に抗菌薬適正使用支援ネットワークを通じて急性期トラストの看護職員に配布した。定量的データを解析し、参加者の人口統計学データと行動を記述した。一方、自由回答形式の質問に対する回答の主題内容分析により、IVOS の障壁と対応策を検討した。
結果
合計 545 名の看護職員が調査に回答した。大半（65.3％）は日常的に臨床医に IVOS を提案していたが、各施設の IVOS 指針を知っていた割合は 50.6％にとどまった。3 分の 1（34.7％）は IVOS を提案せず、医師に頼っていたか、患者に IV 治療が必要であると考えていた、または IVOS を依頼する知識と技能が不足していた。IVOS 実施率を向上させるための提案の内容分析により、3 つの大きな主題（人、プロセス、システム）が提示され、教育およびトレーニング、自信および専門職種間の関係の改善、促進が、重要な要因として特定された。
結論
看護職員は IVOS を他の臨床医に提案しているが、看護職員がこの役割を実現し、自信をもつための、さらなる教育とリソースが必要である。
監訳者コメント ：
近年、看護師の抗菌薬適正使用における活躍の可能性については数多くの報告が出ており、大きく期待されている領域である。抗菌薬の注射から経口へのスイッチに対する看護師の提案はその中でも比較的新しい内容であり、mixed method（混合調査法）によって看護職員の意見も示されており、大変興味深い内容である。
