Burden of surgical management of prosthetic joint infections following hip and knee replacements in Alberta, Canada: an analysis and comparison of two major urban centres

背景

人工股関節および人口膝関節全置換術後における複雑性手術部位感染症（SSI）およびこれらの感染性合併症に対する再置換術は、大きな経済的費用と関連している。

目的

カナダ、アルバータ州における、複雑性股関節または膝関節 SSI に対する一期的および二期的再置換術、デブリードマン、抗菌薬およびインプラント温存（debridement, antibiotics

 and implant retention：DAIR）、ならびにライナー交換を伴う DAIR にかかる費用を評価すること。

方法

Alberta Health Services Infection Prevention and Control データベースを用いて、2012 年 4 月 1 日から 2019 年 3 月 31 日に、アルバータ州の 2 つの大規模都市医療センター、カルガリーおよびエドモントンにおいて、複雑性股関節および膝関節 SSIを有し、外科的介入を受けた 18 歳以上の患者を特定した。マイクロコスティングおよびグロスコスティングを用いて、関節置換術のための初回入院から 12 か月後および 24 か月後の費用を推定した。サブグループ解析、逆ガウス回帰分析およびガンマ回帰分析を用いて、費用と、再置換術費用、年齢、性別および併存疾患との関連を評価した。

結果

全体で、複雑性 SSI 患者 382 例が特定され、平均年齢は 66.1 歳であった。DAIR およびライナー交換を伴う DAIR により、結果として12 か月後および 24 か月後の費用はそれぞれ 53,197 カナダドル（95％信頼区間［CI］38,006 ～ 68,388 カナダドル）および 57,340 カナダドル（95％CI 48,576 ～ 66,105 カナダドル）と最も低かった。二期的再置換術は、最も高額の手技であった。かかった費用の大部分（＞98％）は、初回手技後の 12 か月間に発生した。

結論

医療費は初回関節置換術後の 12 か月間で最も高く、股関節および膝関節の複雑性 SSI に対する二期的再置換術で最も高かった。
