目的

黄色ブドウ球菌(Staphylococcus aureus)中心ライン関連血流感染症(CLABSI)の成人患者ではカテーテル抜去が推奨されているが、長期中心静脈カテーテル(LTCVC)の小児については議論がある。われわれは、S. aureus LTCVC 関連 CLABSI の小児におけるカテーテル救済戦略の実施について評価し、また救済療法失敗の決定因子について推定した。

方法

フランスの 3 次病院 8 施設(2010 年から 2018 年)において、LTCVC を有し、入院して S. aureus CLABSI を発症した小児(18 歳未満)を後向きに組み入れた。カテーテル救済戦略は、菌血症の疑いに対する経験的抗菌薬療法の開始から 72 時間以上 LTCVC が留置されていることと定義した。患者の特性を精査し、多変量ロジスティック回帰を実施して、カテーテル救済戦略の失敗(すなわち、菌血症の持続、再発または合併)に関連する因子を特定した。

結果

S. aureus LTCVC 関連 CLABSI のエピソード 273 件を組み入れた。カテーテル救済戦略は、273 例中 194 例(71%)で選択され、このうち 74 件(38%)で失敗した。カテーテル救済戦略の失敗の主な種類は、菌血症の持続(74 例中 39 例、53%)であった。カテーテル救済戦略の失敗と独立して関連していたのは、カテーテル関連感染症の既往(補正オッズ比[aOR]3.18、95%信頼区間[CI]1.38 ~ 7.36)、トンネル型カフ付き CVC との比較した場合の植込み型静脈アクセスデバイスにおいて発症した CLABSI(aOR 7.61、95%CI 1.98 ~ 29.20)、複数菌による CLABSI(aOR 3.45、95%CI 1.25 ~ 9.50)、ならびに感染症の初期段階における重症敗血症(aOR 4.46、95%CI 1.18 ~ 16.82)であった。

結論

カテーテル救済戦略は、S. aureus LTCVC 関連 CLABSI の小児において高頻度に選択されており、3 分の 1 の症例で失敗が起きていた。特定されたリスク因子は、臨床医がカテーテル救済戦略が失敗するリスクのある小児を特定する助けとなると思われる。

監訳者コメント

黄色ブドウ球菌の中心ライン関連血流感染症(CLABSI)の小児での長期中心静脈カテーテル(LTCVC)の救済療法が失敗するリスク因子の検討。成人ではカテーテル抜去が原則だが、小児では血管アクセスが限られていることや侵襲的処置を最小限に抑えようとすることからカテーテル救済療法が行われている。しかし、今回の研究でもカテーテル救済療法の 3 分の 1 は失敗率であり、失敗率は高い。今後、さらなる検討が必要だが、失敗リスクの高いものについては最初からカテーテル抜去を選択することにより失敗率を低下させることができるだろう。
