集中治療室に入院した患者の多剤耐性/広範囲薬剤耐性アシネトバクター・バウマニー(Acinetobacter baumannii)感染症のリスク因子:システマティックレビューおよびメタアナリシス★

背景
多剤耐性(MDR)および広範囲薬剤耐性(XDR)アシネトバクター・バウマニー(Acinetobacter baumannii)感染症は、臨床治療に問題をもたらし、とくに集中治療室(ICU)において高い死亡率の原因となっている。
目的
ICU に入院した MDR/XDR A. baumannii 感染患者のリスク因子を系統的に要約し、分析すること。
方法
2023 年 10 月までに英語で発表された適格な原著論文を、PubMed、Embase、Web of Science、the Cochrane Library を用いて検索した。メタアナリシスを適宜実施し、連続データおよび名義尺度データについて平均差(MD)とオッズ比(OR)を算出した。対象とした研究の質を Newcastle-Ottawa Scale により評価した。
結果
8 か国の ICU 患者 1,199 例(一般 ICU 604 例、新生児 ICU 435 例、小児 ICU 160 例)について報告した研究 10 報を分析の対象とした。一般 ICU 入院患者において MDR A. baumannii 感染症と関連するリスク因子は、Acute Physiology And Clinical Health II(APACHE II)スコア高値(平均差[MD]7.52[95%信頼区間{CI}3.24 ~ 11.80]、P = 0.0006)、侵襲的手技(OR 3.47[95%CI 1.70 ~ 7.10]、P = 0.0006)、ICU 長期在室(MD 3.40[95%CI 2.94 ~ 3.86]、P < 0.00001)、抗菌薬使用(OR 2.69[95%CI 1.22 ~ 5.94]、P = 0.01)であった。サブグループ解析では、新生児 ICU 長期在室(MD 16.88[95%CI 9.79 ~ 23.97]、P < 0.00001)が、XDR A. baumannii と関連した。
結論
MDR および XDR A. baumannii 感染症のリスクを低減するために、ICU 長期在室、侵襲的手技施行、抗菌薬使用、APACHE II スコア 高値の患者には細心の注意を払うべきである。
監訳者コメント :
多剤耐性アシネトバクター・バウマニー感染症に患者が罹患するリスク因子について、検討を加えている。ICU 長期在室・侵襲的手技施行・抗菌薬使用・APACHE II スコア高値などは、患者が ICU 入室時に評価を行い入院中も定期的に評価を怠らない必要があろう。
