背景

カルバペネムは、重篤な感染症に対して使われる抗菌薬である。カルバペネムの消費量は、微生物の耐性が増加しているために、世界中で増加した。

目的

カルバペネムを制限する抗菌薬適正使用支援プログラム(ASP)の感染性病原体の耐性プロファイル、抗菌薬の使用量、集中治療室(ICU)への入室期間、死亡率、費用の変化に対する効果を調べること。

方法

2020 年 7 月 1 日から 2021 年 5 月 1 日までに ICU に入室した患者を、2 つの期間、すなわち、カルバペネム非制限期間と、感染中にカルバペネムに代わる抗菌薬が選ばれるカルバペネム制限期間に分けた。100 患者日あたりの 1 日規定用量(DDD)を使う方法で、抗菌薬の消費量を計算した。

結果

試験に組み入れられた 572 例の患者のうち、62.2%が男性で、平均年齢は 70.5 歳であった。血液培養で、最も頻度の高かったグラム陰性菌はアシネトバクター・バウマニー(Acinetobacter baumannii)(25%)であった。多剤耐性のA. baumanniiや超多剤耐性のA. baumanniiによる血流感染は、2 つの期間で有意に異なっていた(カルバペネム非制限期間、95.6%[N = 22]、カルバペネム制限期間、66.6%[N = 8]、P = 0.04)。院内感染の罹患密度と発生率が徐々に減少し(P = 0.06)、2 つの期間の間でメロペネムの消費量に有意な減少があった(100 患者日あたりの DDD が、カルバペネム非制限期間とカルバペネム制限期間で、それぞれ、21.19 と 6.37、P = 0.007)。ASP で、抗菌薬の費用が 8,600米ドル 節約された。また、患者の費用全体では患者 1 人当たり 14 %の節約となった(P < 0.05)。

結論

効果的な ASP を包括的な感染制御プログラムと組み合わせることで、抗菌薬耐性菌の出現を減らせるかもしれない。

多剤(超多剤含む)耐性アシネトバクターが非常に多いトルコからの報告。カルバペネムの使用制限を行うことにより、ICU における多剤(超多剤含む)耐性アシネトバクターによる血流感染が減少し、抗菌薬の費用が 140 万弱節約されたとの報告。カルバペネム制限期間においては非制限期間に比べて、コリスチンや ST 合剤を使用していた。トルコでは感染症専門医のみがカルバペネムを処方できるらしく、トルコならではの面白い研究である。
