Impact of the COVID-19 pandemic on the incidence of surgical site infection after orthopaedic surgery: an interrupted time series analysis of the nationwide surveillance database in Japan

背景

COVID-19 パンデミック時に地域社会や病院において衛生意識が高まった。しかし、このような状況が、整形外科手術領域における手術部位感染症（SSI）の発生率に影響したか否かに関しては議論がある。

目的

COVID-19 パンデミックが整形外科手術後の SSI 発生率に及ぼした影響について調査すること。

方法

日本の全国サーベイランスデータベースから、整形外科手術を受けた患者の医療記録を抽出した。主要評価項目は、全 SSI、深部または臓器／体腔 SSI、およびメチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus［MRSA］）による SSI の毎月の発生率とした。パンデミック前（2017 年 1 月から 2020 年 3 月）からパンデミック（2020 年 4 月から 2021 年 6 月）の期間に分割時系列分析を実施した。

結果

計 309,341 件の手術を対象とした。季節調整された分割時系列分析では、全 SSI（率比 0.94［95％信頼区間 0.98 ～ 1.02］）、深部または臓器／体腔 SSI（0.91［0.72 ～ 1.15］）、MRSA による SSI（1.07［0.68 ～ 1.68］）の発生率に有意な変化はなく、いずれのパラメーターにも顕著な勾配変化は認められなかった（それぞれ、1.00［0.98 ～ 1.02］、1.00［0.97 ～ 1.02］、0.98［0.93 ～ 1.03］）。

結論

COVID-19 パンデミックに対する意識と方策は、日本の整形外科手術後の全 SSI、深部または臓器／体腔 SSI、MRSA による SSI の発生率に著しい影響を与えなかった。

COVID-19 パンデミックが、整形外科手術後のさまざまな関連するであろう種類の SSI に関して、大きな影響は与えていなかったという報告である。COVID-19 パンデミックは、整形外科手術にも手術件数等において大きな影響を与える場合があったのでこの報告は、興味深い。
