背景

病院のシンクには、患者に感染する可能性のある細菌が定着することが知られている。

目的

同じ施設内で、既存の新生児集中治療室(NICU)から移設するために新設された NICU のシンクにおける細菌の増殖について調べること。

方法

本研究は、新設 NICU の移転前後において、NICU 内の手洗い用シンクにおける病原性細菌の増殖についての前向き研究であった。様々なシンクのトラップのサンプルを縦断的に培養し、既存 NICU と新設 NICU の間で比較した。

結果

病原性の可能性のある細菌は、新設 NICU の移転から 1 か月以内に速やかにシンクに定着していた。本研究期間中の 2021 年 6 月 29 日から 2022 年 9 月 2 日の間に、サンプル 62 個をシンク 11 箇所から採取し、そのうち 43 個(69.4%)が検査で陽性であった。半定量的細菌増殖スコアの平均は、新設 NICU において調乳室のシンクのほうが患者治療区域よりも有意に高かった(40.67 対 1.768、P = 0.025)。新設 NICU の調乳室のシンクにおける細菌プロファイルは、既存 NICU のプロファイルを反映しており、緑膿菌(Pseudomonas aeruginosa)とエンテロバクター・クロアカエ(Enterobacter cloacae)複合体が多かった。既存 NICUと新設 NICU の両方の手洗い用シンクにおいて、定着していた病原体で最も多かったのはバークホルデリア・セパシア(Burkholderia cepacia)複合体であり、次いでセラチア・マルセッセンス(Serratia marcescens)とエリザベトキンギア(Elizabethkingia)属が多かった。シンク使用の中止により、NICU の手洗い用シンク配水管における細菌のバイオバーデンが有意に減少した。

結論

新設された NICU の手洗い用シンクには、速やかに病原性細菌が定着していた。調乳室のシンクでは、多様かつ多くの病原性細菌の増殖が認められた。

監訳者コメント

シンガポールからの前向きサーベイランス報告。新設 NICU においては特に調乳室のシンクで病原性細菌の定着が速やかに起きていることがわかる。これはミルクやその他の有機廃棄物が細菌の定着を促進していることによる。また、シンクの使用を中止することで病原性細菌が検出されなくなるという結果からは、シンクの使用を中止し、水を使わない患者ケアに移行することが効果的であることを示唆している。
