Vancomycin-resistant Enterococcus faecium: impact of ending screening and isolation in a Danish University hospital

背景

バンコマイシン耐性エンテロコッカス・フェシウム(Enterococcus faecium;VREfm)および バンコマイシ可変性エンテロコッカス・フェシウム(VVEfm*)の発生率が増加し続けていることに対抗するために、世界中の病院で、かなりの資源が使われているが、患者の安全、感染予防、病院の費用のバランスをとることが重要である。

目的

デンマークの Odense University Hospital で VREfm/VVEfmのスクリーニングと隔離を終わらせることが、Odense University Hospital における患者特性、臨床特性、菌血症のリスク、VREfm/vancomycin-variable E. faecium による疾患での死亡率に与える影響について調べること。Odense University Hospital と南デンマーク地域の 3 つの共同研究病院における VREfm/VVEfmによる菌血症の負荷についても調べた。

方法

2015 年から 2022 年の期間に Odense University Hospital と共同研究病院で検出された 1 回目の VREfm/VVEfm臨床分離株(発端分離株)を組み入れた後向きコホート研究を実施した。スクリーニングと隔離を行った介入期間は、2015 年から 2021 年までで、介入後の期間は、2022 年であった。臨床分離株についての情報は、微生物学的データベースから得た。患者データは、病院の記録から得た。

結果

Odense University Hospital では、436 例の患者が研究に組み入れられた。285 例は介入期間で、151 例は介入後の期間であった。スクリーニングと隔離を終わらせた後に、発端分離株数が増加した。van 遺伝子の変異の他には、重要でない小さな変化がその他に調べたパラメーターすべてにおいて検出されたのみであった。30 日以内の死亡率は、VREfm/VVEfmが原因と考えられる死亡を反映していなかった。4 症例においてのみ、死因が VREfm/VVEfmによる感染であった可能性が高かった。

結論

発端分離株数の増加にもかかわらず、短期間の追跡調査中に、スクリーニングと隔離の再導入を支持するものはなかった。
*(監訳者注):VVE(vancomycin-variable Enterococcus)は最初はバンコマイシン感受性として報告されるが、バンコマイシン曝露後に in vivo でバンコマイシン耐性を獲得する腸球菌

監訳者コメント

デンマークの大学病院からの報告。VRE のスクリーニングと隔離を中止することで VREの臨床分離株が増えたが、患者の予後に大きな変化は見られなかった。VRE のスクリーニングと隔離の費用対効果が高くない可能性がある。日本では VRE の検出は極めて少なく、スクリーニングと隔離の影響とその必要性に関してさらなる研究が必要である。
